I understand your concern. Usually the sagging of the scrotum is age related, however if you have recently noticed the sagginess and it is not something that has been developing gradually through the years, then it is most probably either a hydrocele or a varicocele.
A hydrocele is a collection of fluid in the scrotum. Apart from being uncomfortable and unesthetical, it is harmless. Surgery is the solution. Alternatively, fluid draining through pipes and suction can also be done, however this method usually leads to recurrence of the hydrocele and need to be repeated.
A varicocele is an engorgement of veins in the scrotum. It is usually more prominent when standing, usually one sided and almost disappears when lying down. The treatment is through surgery.
