What Causes Severe Abdominal Cramps, irregular Spotting And Heavy Bleeding?
Question: Hi, I have had HEAVY menstral cycles all my life. I had gotten pregnant at 21 and at 34 weeks, I had found out I had severe preeclampsia. My blood pressure skyrocketed to 145/137 and went straight to the ER. They immediately put me on Magnesium and sent me to a bigger hospital with a NICU for our baby. I delivered 3 days later. My baby was healthy, although under weight. I was on blood pressure medicine for 6 weeks. My 6 week exam went well. But about 6 months after, my periods became irregular. I would go one week early, and sometimes 2 weeks late. And I bled very heavy at times, and super light at other times. Then in August, I woke up with severe pain in my pelvic and lower stomach area. It honestly felt like I was having hard contractions all over again. There was so much pressure down there, it felt like someone was pulling out my female organs out of my crotch. I was not on my period at the time, so I had no explanation. It has happened a few times since then. This morning I had started my period and started getting the pain back, but it was worse. Everything went white, and I thought I was going to pass out. The pain only lasts about 30 minutes and comes in 2 to 3 intervals just like contractions. Can you please help! I'm calling an OBGYN this Monday to get an appointment, but I need to know some possibility to what I am facing. Thanks

Hello XXXXXXX
Thanks for writing to us with your health concern.
You could be having endometriosis, that would explain the severe cramps / pain and the irregular spotting and bleeding.
Also, a fibroid or polyp could cause this.
Other than that , it could be simple pelvic infection that leads to adhesions, pain and irregular cycles
You need to have a gynecological exam obviously.
A per speculum and per vaginum internal checkup is essential.
A pelvic ultrasound scan and hormonal levels should be done - FSH, LH, TSH, Prolactin, DHEAS, Free testosterone.
The workup would give a clearer idea of which of these conditions you are really having.
All the best
Please feel free to discuss further.
