What Causes Severe Pain In Buttocks?

Question: One year ago I started having pain in my right buttock after assisting a 350lb patient. I was trying to clean her up in the bed and she was not able to lie on her side and kept rolling back into me. The pain was unbearable when sitting and standing still. Driving a car was almost impossible. I felt as if I was sitting on a bone. I had no pain when walking but when I stopped the pain started immediately. PT was extremely painful. I also felt an extremely burning sensation. Since then it has progressed to include bilateral hips, buttocks, thighs, upper arms, and bilateral hands. On a scale of 1-10 it is often a 9. I tried steroid injection and oral steroids w/no success. I am taking 10/325 Lorcet five times daily w/little relief but w/out it the pain is unbearable. At times I feel as if I am on fire in these areas. I have one point in my buttock where the pain is most intense. Work comp Dr said it was hamstring tendinitis. She later said she could not help me and I would just have to learn to live with it. (She then stated it was "lack of intimate relationship with the Lord." Seriously.) I felt from the beginning it was Periformis Syndrome but the ortho Dr said in his 25 years of practice he has only diagnosed Periformis Syndrome one time and felt that was prob wrong. Physiologist said there was nothing wrong and to go back to work. MRI, x-ray, sed rate, and all test came back negative. Work comp closed my case eight months ago. I have been off work for one year and am going to be terminated soon. I am a nurse and can not work while taking narcotics and also the pain is so severe I could not work. I am 55 years old and up until this injury I had been very healthy. Now I can not exercise and walking any distance makes the pain worse when I stop. When I rise from sitting I have to stand for a moment to relieve the pain enough so I can take a step. I then walk all bent over almost in a squatting position. I feel as if no one believes me since nothing has shown up in the test. I did have a slightly elevated ANA but upon retestng it was negative. I have seen a Rheumatologist and she diagnosed me with leaky gut syndrome. I am at my wits end and just want the pain to stop so I can go back to work.I forgot to mention I now have swelling in bilateral feet, L> R, and bilateral hands. Initial injury was on R side.

Possibility of facet joint arthritis

Hello,

I have studied your case with diligence.

As per your symptoms there is possibility of pain aggravated due to lifting of heavy patient on underlying age related degenerative procedure.

As your MRI and blood reports are normal there is possibility of facet joint arthritis leading to pain which may be missed on MRI which can be better seen on CT scan Spine.

If not relieved then possibility of polyarthritis needs to be considered as you have swelling over hand.
It may be seronegative arthritis which may have test with in normal range.

When such patients come to our hospital I advise them blood test like, ACCP, HLA B27

Many of my patients get relief by yoga pranayama and XXXXXXX XXXXXXX so you can also due that.

I have no back pain. The MRI specially stated no stenosis . I also have pain in front and back of thighs. Any other possibilities? Any possibility of Periformis Syndrome? How about CRPS? When I do a symptom check website it always comes to periformis syndrome. I do have one extremely tender spot in my R center buttock. Absolutely no back pain and steroids gave no relief.

There is no any cause in your history for CRPS.

Another possibility of Sacro iliac joint arthritis.
Associated systemic disease leading to generalized symptoms.
