Question: I am having very bad shoulder pain and it travels down my arm..It is only
On my right side..Also I notice a knot on my neck.I even wake up out of sleep
In very bad pain....I get my heating pad and use ...Eventually it eases the pain

According to the history you probably have a slip disc in the neck and degenerative cervical spine of cervical spondylosis. Most of the changes are age related. This causes pinching of the nerve that travels from the neck to the arm, which is why you have raising pain from the back of the neck and shoulder to the arm and knots.

These knots are muscle spasms which are relieved on heat application. Also avoid forward neck bending and lifting weights. Apply heating pad to reduce the muscle spasm three times a day. Take pain killer and muscle relaxant combination like Tablet Diclofenac and Thicolchicoside combination twice a day for a week.

Kindly get an MRI cervical spine done to know the level of the nerve compression in the neck. Accordingly the definitive treatment can be planned, either conservative treatment with medication and physiotherapy or surgery as and when needed.
