Question: Dear Dr.

I have sever swelling on my fingers of hand and foot due to cold weather, every year it happens but this due to heavy winters it more critical as it is painful as well as lot of itching.

Hello,

Thanks a lot for sending the pictures and details. As I said you have the problem of Perniosis or chilblains. After reviewing the pictures, I can get that you have the same problem and the condition is quite not good in hands.

To tell you simply about the condition: It is a localized inflammatory lesions on acral skin (skin of fingers and toes) which occur as an abnormal reaction to cold in susceptible individuals.

It is quite common in some persons where after exposure to cold there is progressive redness and then swelling of the skin.

This process is due to cold injury to the blood vessels which are contracted because of chilling effect of cold. In perniosis there is a persistent cold-induced constriction of the deep smaller superficial vessels of the skin of finger and toes.

Due to this constriction, the skin is deprived of blood supply temporarily and at that time the skin gets inflamed and swollen which you have presently in the picture.

It is mainly seen due to tendency of an individual and nothing to be worried about. In my patients I guide them to wear warm clothing. Central heating generally prevents perniosis. I suggest you to must avoid cold, damp conditions and should not smoke. Wear double cotton gloves and double layer of socks in hands and feet respectively.

If at all the condition doesn’t get better then I prefer to give anti-inflammatory tablets with tablets which dilates the vessels. I suggest you to meet a dermatologist in case if you are not getting better even after cold avoidance.

thnx for your reply. being female it is not possible for me to wear gloves in both the hands all the time. it is possible for foot only which I m already doing because of which dey r less affected. tell me some ointment that I can apply overnight and can get better off from the situation. though it improves itself with the improvement in weather. but as of now I need some safe medication for relief in hands.

I can understand your concern and also can understand your part of concern as you are female. It is really difficult for a female to manage the things and especially for hands.

In your kind of situation I generally prescribe a pain killer tablet such as Diclofenac for immediate relief from pain. For the application I generally prescribe Heparin Sodium gel (Thrombophob Gel ) to apply at day and night time.

I also suggest you to meet a dermatologist so that certain oral medications can be prescribed to improve the circulation.

Do follow strict cold protection to get earliest and best relief.
