Hello, I am Dr Mody and I would be addressing your concern.
As I start I presume that you are a 47 yr male with only hypothyroidism and on levothyroxine.
You don't have diabetes, hypertension, or any previous heart issue.
Secondly some information whether you are obese, bad lifestyle, whether you smoke and your family history for heart events.
Now in most probable senerio, mostly the feeling is subjective and not directly related to heart as you have the same pounding sensation even while you are resting.
An important reason is your levothyroxine, it's a medication with can increase your heart rate and overall contraction of the heart muscle giving rise to the 'thud' like feeling whenever your heart beats. So it is important to titer is according to the needs of the body, every 6 months S.Tsh and free t3, free t4 for once would be the test to evaluate and titre the dosage. Also over supplementing may cause some tiredness as it increases your overall body metabolism.
Now considering your age, if you have any risk factor as I have mentioned, I would strongly suggest you to get a treadmill stress test, as it will help us evaluate your heart and rule out any underlying cardiac pathology. It's a non Invasive and in expensive test to know reserve cardiac status.
Lastly I would like to impress upon that ibupropin, is a pain killers and has high pospensity to harm your kidney, avoid long use and get s. Creatinine levels done if used more than 3 weeks.
Some supplementation with a multi vitamin and vitamin D will help.
