In case of epigastric pain radiating to the back, acute pancreatitis, M.I and aortic dissection should be ruled out. Unless one knows the amylase, lipase levels, one cant be sure about the diagnosis based on clinical suspicions alone. Treatment should be initiated towards removing precipitants and finding the trigger cause e.g alcohol, NSAIDs, diabetes, gall stones , hyperlipidemia etc. Fluid replacement, analgesia ( tramadol) , antibiotics can be advised accordingly in consultation with your doctor, for now.
