Question: He can not bend over without getting short of breath and sick to his stomach.

I passed carefully through your husband's medical history and would like to explain as follows:

- his actual complaints (shortness of breath and stomach sickness while bending) seem nonspecific for cardiac ischemia, though he seems to have a new coronary artery problem.

Those clinical symptoms exacerbated by certain body posture (discomfort and and difficulty in breathing while bending) are most likely to be related to hiatal hernia with gastro-esophageal reflux.

In such case an appropriate therapy with gastric anti-secretor drugs (such as Omeprazole, Pantoprazole, etc.) could be beneficial at relieving the symptoms.

From the other side, the newly almost plugged artery need to be treated in a timely fashion with eventually a new coronary angioplasty and stent placement.

In addition, some lab test are necessary to investigate for any obvious myocardial ischemia:

- cardiac enzymes (CK-MB, Troponin),
- resting ECG,
- cardiac ultrasound,
- nuclear perfusional stress test

In case an acute myocardial ischemia is concluded by any of the above recommended tests, then it is advisable an immediate intervention with coronary blood flow restoration of that newly narrowed artery.

They did clean his stent out last fall. the bending problem has been going on for over a year. Before the cleaning job and after. I will try and get him to take the Omeprazole. He was on it once but became afraid of it so stop taking it., that probably when the problem started. He doesn't like Drs and rarely tells me anything about his health. Thank You

In addition, some daily life-style modifications would be beneficial at decreasing the likelihood of digestive related complaints.

Avoiding fatty and spicy foods, large quantity meals (instead, frequent smaller portions would be advisable), alcoholic and caffeinated drinks, raising a bit the head of his bed, etc. would be helpful for a more comfortable well-being.

Meanwhile, he should be alert of any cardiac suggestive symptomatology, in order to promptly detect any ongoing cardiac ischemia followed by an optimal treatment strategy.
