HI Doc,
As I stated in my original post I went from a flaccid 7 down to three. And when erect it is still several inches shorter than a year ago.
I do not have diabetes as of two year ago checkup, or Hypertension. I am currently on no medications.

Hi.
Shrinking of the penis requires further workup before treatment is initiated directly. Get a hormone profile to check for testosterone and if there is any need for hormone replacement. Low testosterone can lead to a greatly reduced penis size.
Obesity and having a BMI eg greater than 30 can adversely affect penis size. The duration you described compels for a screening for STDs and urine infection. An ultrasound of penis and groin region for adequate and patent vessels confirmation should also be done.
The pain associated with ejaculation is most likely associated with an infection of the genitourinary system samples of urine or semen should be tested. Antibiotics would be required if infection is suspected. An enlarged prostate should also be ruled out on physical and on ultra sound.
Lastly Psychiatric issues may also be responsible esp perception issues with penis size.I would suggest you consult a psychiatrist
so that a XXXXXXX mental state exam is performed.
to ascertain depression and or anxiety,
Meds can be adjusted afterwards in addition
to cognitive behavioral therapy.
Let me know if you have any query
wishing you best of health
thanks
