Brief Answer:
likely STD, investigate and treat

Detailed Answer:
Hi XXXX
Welcome back to health care magic.
The symptoms you are suffering in penis skin are most likely a reflection of of a Sexually transmitted disease.
It can be troublesome problem and a serious health issue because it is persisting and recurring aggressively.
You should get a complete blood count and Gram stain and culture of the wound/blister swab, when they are present.
Also test your blood for VDRL, markers for HIV, hepatitis B and C.
You should get examined by a local physician.
Most likely you will need treatment with prolonged antibiotics course and local application of antiseptics and cleaning of the local area to maintain a good local hygiene.
I hope the advice will be helpful for you.
