Hello, greetings!
Thanks for posting your query on hcm.
I have seen your attached photo. You do seem a bit drowsy, like you haven't had a good night's sleep or maybe you are tired.
You have to remove any kind of fear and anxiety about fatal familial insomnia that you may have.
Let me know since how long have you been sleepless and since when is this going on so that I can assist you better?

Brief Answer:
Polysomnography and investigation required.
Detailed Answer:
It seems like you have been diagnosed with anxiety, which is one of the findings in familial fatal insomnia. Also, you are suffering from insomnia since couple of months which seems like it is rapidly progressing.
If you are on any medications for anxiety or insomnia, do let me know.
In your case, before any treatment it is important to diagnose the condition.
-You should undergo a polysomnography or a sleep study
- Also, a Positron emission tomography will be helpful to diagnose this disorder.
- Your symptoms need to be charted down for a span of a few weeks for final diagnosis of familial fatal insomnia.
I would like to put forward this point that there is no definitive cure of familial fatal insomnia. Drug trails include usage of quinacrine, doxycycline, and intracerebral pentosan polysulfate (PPS).
Reduction in anxiety and stress will definitely help.
I hope you found this answer useful.
If you have further doubts/ clarifications, you may get back to me.

Dr. Sonia

Brief Answer:
Anxiety needs to be treated
Detailed Answer:
You must consult a psychiatrist for your anxiety. A few sessions of counselling combined with medication will help you a lot. You may need to take anti anxiety drugs belonging to the class of benzodiazepines. These are prescription drugs and you may get a prescription from your local psychiatrist.
Once your anxiety is taken care of, then we may deal with other problems.
Thus, I recommend that you start treatment for your anxiety without further ado.

Dr. Sonia

Brief Answer:
drug regime needs change
Detailed Answer:
Your heart rate seems low. Did you have a history of high blood pressure? Do you have any documentation regarding any heart issue? If so, please share. You are on lorazepam but this does not seem sufficient as you are still suffering from anxiety and insomnia. Most probably you need to add another class of drug like selective serotonin reuptake inhibitors (SSRIs), tricyclic antidepressants (TCAs) in your regime.
This has to be done after consultation with your psychiatrist. Thus I recommend you see him so that your drugs can be modified accordingly.

Brief Answer:
Change of medication regime needed.
Detailed Answer:
As mention earlier, you need a change of anti anxiety medication. Along with Lorazepam a selective serotonin reuptake inhibitor (SSRIs) or tricyclic antidepressant (TCAs) has to be added. This will help you in getting adequate sleep and will also bring down your anxiety levels. However as these are prescription drugs, you need to get a prescription from your psychiatrist.
