Question: I am a 19 year old boy with a throat problem. My throat is coated with a white substance which is uncomfortable all the time and is very sore when I swallow. I have had an off and on low fever, am on amocicillin for two days, but the throat problem does not seem to be improving. What do I have and what do I need to do to get rid of it.
Forgot to mention, the doctor did a strep test and that came up negative (no strep)

Brief Answer:
Need to take complete course of antibiotics rather than just 2 doses.

Detailed Answer:
Hi,
Welcome to health care magic,

I have read your whole query and will be answered by me.

Sore throat, mild fever with difficulty in swallowing are symptoms of upper respiratory infection.

Majority of cases of throat pain are due to viral infection, but in some cases it is related to bacterial infection (like streptococcal pharyngitis).

You have already ruled out bacterial cause by doing strep test. So,in your case it would be probably viral origin.

If I would have your treating doctor,I would like to give,

1) Complete antibiotic course with sufficient dosage like you are taking amoxicillin since two days,I would prefer to give amoxicillin with clavulanic acid (500+125 mg) three times daily for minimum 5 to 7 days complete course.

You have taken only 2 days doses of amoxicillin and felt no improvement after that, So it would be advisable to go for complete course of antibiotics.

However, antibiotics are not usually required in case of viral infection but if you would take it then there will be no issue, rather it would certainly help you to resolve from this throat symptoms quickly.

Also, sometimes it might take more than couple of days to improve from the throat symptoms,so better to continue same treatment along with supportive treatment.

As antibiotic is a prescription drug, you can discuss with your treating doctor before acting upon. I am sure you symptoms will start subsiding once you will complete the whole course of antibiotic treatment.

Apart from antibiotics, you should also take symptomatic medications,

2) take pain killers with anti-inflammatory combination (paracetamol 500 + ibuprofen 400) drugs available over the counter.

3) Antihistamines like levocetrizine or benadryl if you have a dry cough or cold.

4) Nebulization with steam inhalation 2 to 3 times daily and warm salt water gargles which will also be helpful for symptomatic relief.

5) Take multivitamins like vitamin B complex with folic acid daily once.

I would expect you will start improving with initial 3 or 4 days dosages but again I would say not to stop the antibiotic course, rather you should complete it and most probably your symptoms will not reappear.

Hope this will help you, feel free to ask if you have more questions.

Thanks and Regards,

Thanks for the info; very helpful.

One additional question. Why is my throat covered with the white substance. Is that common for a throat virus; have had a sore throat before, but don't remember my parents telling me my throat had that white aspect.

Brief Answer:
It is associated with throat infection.

Detailed Answer:
Hello,
Thanks for the reply.

Yes,white coated throat is common (but not always) in case of throat infection and it would also clear up with the infection as you will complete the treatment,so no need to worry about that.
