Brief Answer:
Referred pain of esophagitis.

Detailed Answer:
Hi ,
Thanks for asking query on this forum.
Have gone through your details and i appreciate your concern.

Most likely cause of this shifting soreness and pain is esophagitis. When acid in stomach refluxes back into esophagus it erodes the mucosal lining of esophagus causing inflammation and pain.If left untreated and in long standing cases this pain can radiate to back and chest also. The dermatomes which suply to back and chest are sme so pain of esophagitis can be felt in thoracic and back area also.

Secondly retrosternal chest bone is classical of heartburn due to esophagitis.

Suggestion
Start taking a proton pump inhibitor like Omeprazole 40 mg once dailt along with prokinetic drug like Levosulpiride(prescription medicine). This will provide you with immediate relief.
An antacid should be taken thrice daily to neutralize the excess acid regurgitating back into esophagus.

Follow both the suggestion and pain should respond immediately to them.
Hope i answered your query well.
Follow up are welcome.

Detailed Answer:
Hello again,
Gabapentin 2400mg per day is a very high dose and most ligely cause of heartburn and acid reflux in your case can e such high doses of gabapentin.
Omeprazole 40mg is just a baseline dose.If its not that effective consider adding levosulpiride(prescription med) and shift from omeprazole to rabeprazole once daily.
An antacid can be taken thrice daily before breakfast lunch and dinner to relieve you from symptoms of heartburn.

However as you are 67 and chest is involved few baseline tests of cardiology should also be done to rule out possible cardiac involvement.

Hope the answer helped you.
Get well soon.
Regards.

yes, it has helped. I take the gabapentine for neuropathy. I started at a lower dosage but had to increase. It's funny how this pain moves. yesterday it was by my left scapula and today behind my right breast. of course, my upper back still aches, but the pain is more behind right breast. also, I get full awful fast when I eat and get very uncomfortable. I don't eat much either

Detailed Answer:
Hi again Dear,
All your symptoms point towards a mix of gastritis,Dyspepsia and Acid reflux.

The unpleasant sensation like prolonged persistence of food in the stomach after eating is called post prandial fullness and functional dyspepsia is one of the most common cause of it.The primary aim of the stomach is to store food. The stomach contains hydrochloric acid which breaks down proteins and activates enzymes which act on the ingested food and digest it.In situations where intragastric content rapidly flows into the duodenum food is not broken and digested properly hence causing Fullnes dyspepsia and heartburn.

As explained above as pain is involving thorax initially tests related to heart should be done.Once cleared from cardiac side follow treatment of acid reflux and dyspepsia.

You would feel better soon.
Thanks you and Regards.

Detailed Answer:
Hi dear,
Hope you are feeling better now.
As explained earlier Heartburn is a symptom and not a disease. When acid irritates the inflammed mucosa nerve cells inside the mucosa carry signal toward brain saying there is something wrong. This signal is interpretated by brain as a sensation referred medically as heartburn. When you take antacid or Omeprazole acid secretion is neutralised hence sensation of heartburn goes off.Heartburn is just a symptom of reflux disorder.

We have to rule out each possibility one by one lets start with cardiac initially.Once cleared from cardiac side lets investigate it as gastric case.In case everything turns out to be normal again then we can consider this as muscular pain and treat it accordingly.
However if you feel like pain is musular easiest way is take Tylenol(OTC) twice daily for 3 days.Muscular pain would definitely respond to anti inflammatory.

Regards.

Detailed Answer:
Hi dear,
Tylenol contains acetaminophen which is a cox3 inhibitor and a very week anti inflammatory.Don't worry it will not aggrevate your gastritis.
Ekg will not do atleast get a stress test or 2d echo cardiogram.

hi. really had the heartburn this morning. took the omeprazole plus 2 tums. yikes!

Answered by Dr. Sunil (31 minutes later)
Brief Answer:
Take Rabeprazole with prokinetic drug.

Detailed Answer:
Hu again dear,
Sorry to hear about that.
Please ask your Gastro to give a prescription of Rabeprazole 20 mg once daily along with levosulpiride for 2 week.
You will get a much faster and long term relief.
As already told by me all your problems are somehow related to acid disorder.
Omeprazole is a week proton pump inhibitor.Secondly it has no effect on acid reflux( responsible for heartburn).

So take a combination of Proton pump inhibitor and Prokinetic drug as adviced.Proton pump inhibitor would prevent acid formation while Levosulpiride would inhibit its reflux into esophagus.

Hope you get well soon.

I sure am a mess, aren't I?
Answer-
"If I lose my right arm, how do I know I don’t need two arms? …The story, ‘I need two arms’ is where the suffering begins, because it argues with reality. Without the story, I have everything I need. I’m complete with no right arm."Be thankful to God that you are a citizen of developed country,There are millions of people(in my country) who don't have even primary doctor for consultation. Be confident from inside & be positive , your inner strength is of utmost
importance to keep your symptoms under control.Keep laughing from deepest corners of your heart & see the life from new
angle as every rising sun gives new rays of hopes for everyone.Every person is important in this world. You too are important for your friends to your family to your beloved and to me(as my patient).
I would just like to give you a personal tip(I use to follow it when i was depressed)-When ever you feel that your ailments are making you depressed visit the nearest emergency and trauma centre. Spend there half an hour,Believe me when you would be leaving hospital no matter how ill you are you would be thankful to god.

Lord XXXXXXX in Bhagavad XXXXXXX compares the human body to a piece of cloth. A person should identify not with the body but the real Self within. One should not grieve for growing old or having a disease that cannot be cured. Just as worn out clothes are replaced by the new ones, the soul of a person acquires a new body. Identification with the Self instead of the body helps a seeker to be detached from the limitations of the human body. Releasing oneself from worries and all other negative emotions is easy by surrendering oneself to the almighty. Since a human being is only an instrument in the hands of God, regretting the past or fearing the future is pointless. The natural harmony of mind and soul is restored by recognizing the Omnipresent.
Just be patient, this malaise that ails you will go away, then you will be with a smile on your face.

Regards.
