What Causes Spotting At The End Of Ovulation While On IUD?

Question: Hi, I'm 30 years old and had copper IUD fitted 2 years ago. Since then I have been having bleedings or spotting at the end of my ovulation. I had a scan that showed the endometrium at 7.6 mm on 10th day of cycle. I was told I was ovulating and that there are no polyps, fibroids or fluid in the uterus and that the ovaries are smoth. I'm only worried about the lining being thick should I be concerned even though my doctors are not? The ultrasound technitian said she cannot comment on the endomtrium since I have an IUD. Is that true?

It is normal for the endometrium to be thick in such a scenario. The endometrium proliferates in anticipation of receiving a fertilized ovum for implantation; if it does not arrive, the endometrium regresses and is finally shed with menstruation. There is no cause to be concerned.

An IUD can cast shadows and may make it difficult to assess the endometrium.

Thank you Dr Saha, that's reassuring! I am going to have a follow-up visit today and still have to do the hormone level tests but can a endometrial hyperplasia or even cancer be excluded at this point? I'm only worried because I have mid cycle bleedings pains during ovulation and before my period (sometimes even a week before it). My periods last 6-7 days and because of all that I can see blood in my mucus to some extent almost every day of the cycle.

I do not think that it signifies cancer.

Thanks for writing back. Mid cycle bleeding is not normal and needs to be investigated. The follow up visit and the hormone level assay should help. I am not thinking of cancer at the moment. For cancer to be detected a mass or tumor needs to be present and unless that is detected you need not worry.
