What Causes Squeezing Feeling Around Throat And Sensation Of Post-nasal Drip?
I am having problems with my throat. it feels like occasionally like I am being strangled and then a post nasal drip feeling happens. I thought it was GERD but after my Barium Swallow test they found my GI to be normal but that I have cervical degeneration. Can the two be related and if so what do I do about this? I need some relief. I hate this choking feeling. Thanks in advance.
It may be GERD. You need more aggressive testing and treatment.
Detailed Answer:
Dr. Hanif warmly Welcomes you to my virtual clinic HCM!
I have carefully worked through your case, and can realize your health worries. Being your physician, I will take care all of your medical concerns.
Acid reflux and GERD are difficult to differentiate in most patients as they are very common.
Barium Swallow can only detect one third or less patients of GERD correctly as only few have developed the esophageal changes in esophagus to be detected on Radiograph of barium swallow test. Others skip this diagnostic procedure.
Then we have other methods for testing GERD. Just to list some of them they are:
Gastro- and Esophago-scopy.
Biopsy of esophagus.
Esophageal Manometry.
Impedance testing.
24 hour pH monitoring of esophagus.
Management options:-
1.H.Pylori are a very important cause of the Acid reflux disease. If you missed the Barium swallow radiograph, you be having this cause predominantly. Anyway MY TOP DIAGNOSIS for you is GERD and If I was your treating physician I would have prescribed you with Triple Therapy for H. Pylori infection, IF YOU WERE POSITIVE FOR H.PYLORI.
2.If negative for h.pylori but positive for GERD with any of other testing method, I would have prescribed you with Dexilant and Ondansetron and Azithromycin if not allergic to.
These two management options are best for you and you will get better soon. You may consult your treating physician to reinforce.
Yes Cervical Degenerative Disc Disease is linked to your condition in a way that such painful and stressful conditions raises the Acid production in the stomach and cause the symptoms worse. Acid when comes in throat, it irritates the mucosa and causes post nasal drip (PND) foreign body sensations in throat and vocal cords irritations and upper respiratory tract infections.

The Prilosec prescription is not really working because I still get the feeling. It is not constant, but comes and goes.
I do feel the sensation when I turn my neck, I feel the choking sensation (strangling feeling) on the upper portion of my neck right below the chin. (lymph nodes areas?)
Prilosec was not working thats why I suggested more potent ones.
Detailed Answer:
Welcome back.
Prilosec was ineffective, That's why I added more potent one the Dexilant. And I added Ondansetron and Azithro too. Yes you should consult first your Orthopedist to fix the cervical issues.
The chronic GERD also causes the strangulation feeling in the same area as described by you.

how do I get a prescription for the medications you suggested? what are the side effects? I take citalopram. Are there and drug interactions?
No pharmacological interactions they have.
Detailed Answer:
Welcome back!
I have not given a 100% diagnosis that you have GERD. You have possibility of GERD that many physicians miss as the diagnosis and is very common disease.
I would first test you for h.pylori; if positive will for its treatment aforementioned. If negative I will go for GERD testing other than Barium Swallow Radiography. If positive I will go for the drugs I named above in my treatment section.
And if all the GERD tests go negative then I would think of other reasons and they include:
Cervical degenerative disc disease.
Thyroid Disease.
As you are taking Citalopram, It is a drug that has a vast variety of drug pharmacological interactions and usually increase QT Interval on EKG of heart. So, if you get positive for GERD after testing I would definitely like to give you alternatives along with Citalopram.
Amoxicillin instead of Azithromycin for a full course.
I will omit Ondansetron, and if highly needed then I would rather give Nabilone as an alternative at low dosage only for short time.
