Question: i have chest pains shooting into my arms numbness sometimes the pain is a 5 out of 10 and i get stabbing pain that makes it go to a 10 i have had multiple ekg blood work and ct scans done and they tell me they dont no what the problem is i just had my gallbladder removed 2 months because if gallstones inflammed gallbladder i also have oesophagitis do to oral thrush candida albicans i have been tested for Hepatitis and hiv with negative results my Bilirubin is at a 3.0 i have enclosed my medical results i need some input as what this could be am i suffering liver problems with my bilirubin being so high i have enclosed ct scan results with contrast

I have gone through your query.Thanks for using HCM.
The chest pain which you described appears to be due to oesophagitis.You already have oesophageal candidiasis .please complete course of antifungal and take PPI like Rabeprazole.
I would like to know whether you take alcohol ?If yes since how long and what is your regular consumption?
you have fairly longstanding data since NOv.2010.Kindly furnish your full history so that I can guide you properly.please get direct and indirect fragments of Bilirubin checked so we can get clue regarding its cause.
You have a pulmonary nodule for sometime.It is likely to be benign lesion ,but if persistent then you need to discuss with your treating doctor for possible need for FNAC i.e fine needle aspiration for cytology examination.
I would be glad to guide you further once I get your feedback

I drink alchole sometimes a beer here and there with a meal I'm not a heavy drinker and hardly drink the nodule had been same size since last year the Dr told me it could be calcium buildup I actually have 2 nodules left side of lung but want to make sure and get a second opion dont want to chance anything the problem with antifungal medication is its hard on the liver my family dr don't want me taking anything but nyastatin becuse of bilirubin being 3.0 will this this treat the swollen oesophagus or do I need something else my toung was coated white but lately I been taking daily probiotics and brushing my toung with hydrogen proxide toothpaste and mouth wash most of the white has gone but the backside of my toung still has it and it feels like something is stuck down my throat do to the oesophagus being swallon any input would help as to what I should do thanks

Hello,
From your description it is quite clear that you have extensive fungal infection right from throat till oesophagus. Your liver enzymes are normal. I wouldn't avoid antifungal medicines with normal liver enzymes though you have elevated bilirubin. But perhaps I would get culture for your fungus to choose right medicine.

It is also advisable get your direct and indirect bilirubin checked along with evaluation of pulmonary nodule (whether it is related to a fungal lesion). Further we must rule out immunocompromised state and if needed please repeat your HIV test.

Please avoid alcohol totally.

I did get a throat culture it came back as Candida albicans dubliniensis a nurse stuck her self with a needle taking my blood at the hospital so they had to give me a rapid hiv test and they said it came back normal what medication would you recommend I take

check your culture report ,it will show which medicines are most effective against this candida.You should select antifungal from that report.You should get HIV by ELISA technique to rule out HIV.
