What Causes Stomach Cramps Along With Diarrhea And Back Pain?

Question: Yesterday I had intermittent cramps, with diarrhea. The cramps were intense. I had a fever (102.4). My back also hurt - lower side-back on each side. This morning and today, no fever but abdominal pain when I touch or press on my abdomen (anywhere on it, even on the sides). The pain in my abdomen even occurs with each step I take. I a burping a lot too. I'm thinking this is gas, but now my lower chest is starting to hurt. Is this something I can 'ride out' (gas) or should I see a physician. I have GERD; but these symptoms are nothing like previous GERD symptoms.

Hi XXXXXX ! Good morning. I am Dr Shareef, a general surgeon answering your query.

From your history, it seems that you might have got some kind of acute intra peritoneal inflammation for which there could be many reasons, specially with your history of taking tylenol (that too if it was ingested in empty stomach). If I were your doctor, with this kind of pain I would advise you to report in the ER of the nearest hospital, where they could admit you to rule out a peritonitis due to ?perforation or any other organic cause and perform some investigations like x-ray of abdomen in upright posture with diaphragm, an ultrasound of abdomen and a few blood tests like a CBC, serum amylase and lipase, a LFT and KFT. Further management would depend on the result of clinical assessment and reports of related investigations.
