Question: hi; i have had shoulder blade annoyance under ribs on the right side. it first began in january of 2015.
it has not prevented me from doing anything.
sometimes i feel it in my kidney area.
not really painful, just feel something there.
this has not prevented me from living my normal/active life as a football coach.

i have had lots of gas and stomach rumblings.
i have never been gassy like this.

i have had unexplained weight gain, and more unusual is the inability to lose the weight, despite being an avid runner, I ran 2 20k races this spring, 2 10k races and i live a very active life.
i usually have no trouble losing any weight i gain and now it is all in my lower belly.
never had this kind of belly fat and my diet/lifestyle has not changed.


i am constipated, but when i have to go, i have to go immediately, like sometimes a race to the toilet.

i have been unusually lethargic and lazy.

tough getting out of bed because i'm tired and unmotivated.
i've been unmotivated at work and at home...feeling lazy.

i feel something is amiss and haven't felt like myself.

Considering the age and the history that you have provided my thoughts are:
Since you are a football coach with history of active life in past the symptoms should be investigated for in the following way:
- Tests of blood: routine and particularly for diabetes including HbA1c, thyroid and liver functions.
Tests of stool and urine
- Ultrasonography of the abdomen
- Clinical evaluation and examination by a Doctor to have clinical judgement and to correlate with your test reports and advise for further tests depending upon the clinical findings and to refer to the Specialist if required.
- With the advancing age, there can be some usual issues that you might not have been prepared for (just a thought); you may please share if you have and you can.

so what explains the weight gain and inability to lose weight?
i still have a hearty appetite.
i drink lots of water.
am fatigued, but not too tired to run 20k races, 10k rases and coach a football team.

To know the reasons for weight gain, inability to loose weight in spite of such an active life, you need to undergo clinical evaluation and the investigations as suggested above in details.
It is difficult to understand that a person who is a football coach and not too tired to run 20K races feels fatigued, feels lethargic and lazy, tired and unmotivated. There comes the angle of some issues if all the tests are normal.
I did not say old. Advancing age is one thing that everyone goes through and has its own usual changes, challenges, may be some sort of stress and/or anxiety. You alone can know as you have not mentioned anything about this part, which I am trying to explore as one of the commonest reasons we find with.
Please get all the tests done as mentioned above and give feedback.
The response/answer totally depends upon the history and the feedback you give. Hence the need of detailed history and feedback.

through the "research" i've done the past 19 months, i was thinking this was something with the gallbladder.
i have no symptoms/issues when i am active, coaching, running, exercising; only when i am sitting at desk or relaxing, watching television.
i had a full physical examination in december of 2015 and all bloodwork was good/normal, although i did not mention these symptoms at the time. when this first began in january of 2015 i thought it was a pulled muscle in right scapula region. strength and endurance are still good. appetite still good.
my job comes with a certain amount of stress, but do not feel this is stress related, but what do i know? i am no doctor?
are there other questions you would like to ask me about condition or history? i plan to see my GP after the July 4th holiday.

19 months: research you have done indicated something with gall bladder - no symptoms when active - only if relaxing/sitting at desk, watching TV - that is when you are not actively involved into some activity actively - have stress at job...

All this history again indicates towards the probable cause related to stress and related issues I wanted to say in the previous answers.

If this was due to gall bladder problem, there would have a lot of pain particularly after fatty meals. Ultrasound and related further tests would help to confirm or rule out the causes.

Stress can cause fibromyalgia, causing the typical shoulder blade annoyance under ribs on the right side which is not preventing you from living normal/active life as a football coach and all the other symptoms of gas, stomach rumbling, has to rush for passing bowel movement, lethargy, and other symptoms that you have.

Stress and related factors always have the effects.

Since you are about to consult your GP, please discuss these issues as well and get the investigations suggested above.
