Brief Answer:
Biopsy of enlarged gland is recommended

Detailed Answer:
Hello
It’s good to see you
After going through your suffering carefully, if the swelling in the neck region are not responding to any treatment (antibiotics), then my suggestion would be to have biopsy of the enlarged gland, it can be done by either FNAC (fine needle aspiration cytology) or by open excision biopsy after a thorough physical examination. This is usually an OPD procedure that is you will not require a 24 hour admission.
I may assure you this is the best way to reach the diagnosis and plan further treatment.
Finally I apologise for misreading your backache problem, but you may follow the suggestion given this will help you.
I shall be glad to answer any further query.
Take care
