Detailed Answer:
Thank you for asking!
What you are having is the adverse effect of the aesthetic product poly-L-lactic acid/carboxymethyl cellulose/mannitol also called Sculptra. This is called an injection site complications including of either swelling or erythema or inflammation or hemorrhage or nodule or papule.
The odds for it are increased more due to the immunocompromised status due to the HIV Meds you are currently using superadded by history of hypertension for which you use hydrochlorothiazide.Plus use for years is also a reason for this adverse effect.
In nutshell, This is the complication of the injection site of sculptra and you better seek your cosmetic surgeon to fix it, i would suggest stop this aesthetic injections and get the consultants doctors management for this inflammation. Immunocompromised status and history of hypertension makes it worth management. Stopping the product would be the best thing right now.The swelling will subside with some anti inflammatory management. get to your cosmetician and let them fix it for you.
I hope it helps.Don't forget to close the discussion please.
May the odds be ever in your favour.
