Question: My grandson has a swollen lymph node in his neck. He is on a strong antibiotic because streph was suspected. Streph test was negative. He continues to run a fever. Spiked last night to 102. He is five days out on his antibiotics and he complains of headaches when he stands up. Any suggestions?'

Brief Answer:
As detailed below.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Male/6 - grandson - swollen lymph node in the neck - strep suspected hence on strong antibiotic - but strep neg - continues with fever - spiked to 102 last night - on antibiotics for 5 days and Motrin - headache on standing up ... wants suggestions ...

As per the history you have provided I would suggest you the following:
- Blood tests: CBC, blood culture and sensitivity, and other tests as per the clinical evaluation by a Pediatrician.
- Throat swab for gram staining, culture and sensitivity.
- Urine test
- X-ray chest.
- Any other tests as per the clinical evaluation.

Change over to another antibiotic as he is already on the same antibiotic for 5 days and then change over to another if the reports suggest so.

Ultrasonography of the neck to see for any internal change in the lymph nodes like liquefaction and FNAC means fine needle aspiration cytology.

I hope this answer helps your grandson, please feel free to ask for further relevant queries if you feel that there is a gap of communication
