Syncope warrants a cardiac workup though a neurological assessment would also be required due to the history of head trauma and urinary incontinence on regaining consciousness.
Based on your history, it appears to be a concussion and a CT scan should be done in consult with your doctor to rule out any serious brain injury. The CT should have been decided upon as soon as you got injured. However if it was deferred you should go to an E.R for further assessment. If there are no signs and symptoms of elevated intracranial pressure then pain medication can be adjusted accordingly to give adequate relief. The post concussion phase can last up to three weeks including symptoms like urinary incontinence before resolution of symptoms so if the CT is clear then you can relax. If the incontinence persists after 3 weeks or if the episodes are associated with violent jerky movements , tongue bite etc then seizure workup should be considered as seizures can occur secondary to traumatic brain injuries. EEG and Anti seizure medications etc would be decided upon after consulting your doctor.
Transient drop in blood pressure occurring when one stands up from a seated or lying position (orthostatic hypotension) is a common cause of an isolated episode with rapid recovery upon normalization of blood flow. Hypoglycemia and electrolyte imbalance should be worked up. History of any sedatives or other medications related to the syncope should also be looked for. Blood pressure and metabolic profile including Blood sugar and electrolytes needs to be checked.
