Brief Answer:
Need more information for further comments.
Detailed Answer:
Hi,
Welcome to health care magic,
Your nasal endoscopy report is normal as stated by you,so there will be no nasal polyp, deviated nasal septum, swollen turbinates or any congestion.
Here,I would like to ask the exact clinical symptoms which you are suffering from this 3 years like excess sneezing or nasal blockage or allergic rhinitis.
As you are already taking nasal spray and antihistamines,I guess you must be having symptoms of rhinitis.
Kindly get back to me with some more nasal symptoms which you are suffering and if possible kindly send this nasal endoscopy report.
Regards,

Brief Answer:
It will require further ENT evaluation.
Detailed Answer:
Hi,
Thanks for the follow-up.
I have read your whole query and understand the concern.
Here, you are suffering from tension nose but still you have not mentioned any specific symptoms for which you were taking xylometazoline nasal spray and antihistamines.
Anyway, tension nose is a combination features of protruding tip and nose protrusion under the upper lip. Many patients do not have any significant clinical features.
The causes could be overgrowth of the nasal septum leads to tip drooping down and short upper lip.
This can be diagnosed clinically or on the basis of some X-ray and Scan of the face by the ENT specialist.
You might require septoplasty or rhinoplasty after conducting clinical examination and assessing clinical features. The ENT specialist would be the best doctor who can guide you in this regard.
The above operation would be the best and permanent solution if you have this tension nose problem well diagnosed clinically.
Hope this will help you.

Brief Answer:
See an experienced ENT surgeon or plastic surgeon.
Detailed Answer:
Hi,
Thanks for the follow-up.
The treatment you have take so far like xylometazoline spray or drops, nasal saline and antihistamines, are all symptomatic treatment for your nasal obstruction or tension nose because of minimal hypertrophy of turbinate.
We need to find out the cause of this tension nose which could be overgrowth of the nasal septum here but it must be evaluated by clinical examination of nose and some tests as I mentioned earlier.
In my advice, you should consult your ENT department head or plastic surgeon who will examine the nasal cavity with some relevant investigations to find out the actual cause.
Septoplasty or rhinoplasty would be the best and permanent solution for your tension nose problem,if it is very well diagnosed.
In case,no exact cause is found then you should go with symptomatic treatment.
