What Causes Throbbing Sensation In The Testicular Area?

Question: Hello,

I have a chronic complaint for the last 30 years, I have gone to many local doctors over the years who all of them have not fully understood my issue and prescribed general pain management medications that brought no relief.

Complaint description:

sensation of pulling, some sort of contraction and throbbing (pulsation) in the region of the testicles internally (can't point it), it comes when I lay on bed preparing to sleep for a duration of 1 to 2 hours, it doesn't hurt but it makes me very uncomfortable and prevents me from sleeping for the duration.

Additional information:

One hard to explain characteristic regarding this issue is that when I am injured, let's say for example I have a wound on my finger when the issue starts the pain in my finger increases dramatically. I was able to live with this issue in my youth years up to recently, till the last 2 years I started to have a chronic upper back pain that gets unbearable when the issue starts. I don't really understand the origin of such problem, but I deeply believe that a neurologist will have a greater understanding in comparison to other specialties. I was once given some muscle relaxant injection for my chronic back pain and it completely stopped my testicle issue. However, I was told that such medication is not made to be used for long-term treatment.

I hope everything regarding my issue is clear. if there is any additional information needed or anything is not fully addressed please do not hesitate to ask.

Regarding to your description of your concerns and relief after muscle relaxer drugs, it is more likely a muscular condition that is causing your problems.

However, it is necessary to provide more information about examinations you have been through and their results, in order to understand better your condition. (imaging studies, blood tests, urinalysis etc.)

Also, if you remember any kind of trauma in the past, please provide the mechanism(s).

Thanks for the quick reply to my concern,

Regarding diagnostic tests I haven't done any imaging scans but I may do ultrasound as soon as I get time to. My blood tests and organs function is totally healthy. Urine test shows some blood in the urine that is related to recent prostate congestion. I remember that none of the doctors I visited have noticed anything abnormal in physical examination to the region.

I do not remember and I don't think I had any direct or indirect trauma to the region or anywhere around it. However I seem to remember an activity that somehow synchronized with the onset of this condition, in the past I used to run for 30-60 min wearing tight athletic pants that with fraction make the pubic region very hot with.

What concerns me the most is that how a local muscular condition can affect the whole body the way I explained, would you mind explaining such thing ? I was able to live with it long enough till I started to have chronic back pain that increases by three or even four times the normal rate as this condition starts.

In this case, in my opinion, a muscular condition, for example a chronic prolonged muscular inflammation of the muscles of the pelvic floor should be considered and evaluated further.

An abscess for example may cause in latter stages, muscular scar tissue.

It i necessary to get a MRI of the pelvis in order to understand better your condition.

Lowering of the pain threshold with chronic back pain may point towards conditions such fibromyalgia.

I think you should discuss with your Doctor about getting evaluated further for fibromyalgia too.

Thank you very much for the reply

I finally got a lead or at least an educated guess to start an insightful investigation

Now and for the next two weeks, I am in an area with no advance medical care or hospital with subspecialties. Can you prescribe me any sort of medication that may help to ease my condition till I get access to a better medical care ?

You can use Tramadol 50 mg thrice/ day for example, to ease your pain.
ow doses of amytriptiline before sleep (10 mg) also may help with pain and with sleep.

These are prescription drugs so, it is necessary to discuss with your Doctor about their use.
