What Causes Tightness In The Neck Muscles?

cardiac cause to be ruled out, ENT examination further
Detailed Answer:
Hello and welcome
I appreciate your concern
Based on your history and description, it's likely to be related to a gastrointestinal cause like GERD (GastroEsophageal Reflux Disease).
If the spasm/tightness or pain involves the neck and jaw then ideally a cardiac cause for the symptoms should be ruled out by doing a simple EKG to look for any new changes. If the EKG is normal with no evidence of ischemia or injury then PPIs/antacids (eg. Omeprazole, Pantoprazole etc.) with prokinetics can be advised based on clinical assessment.
The throat should be examined along with a neurological exam that includes testing for the gag reflex and an ENT consultation may also be recommended.
