There could be some inflammation / temperature changes in the spermatic cord or the scrotal skin; but this is not STD...

Brief Answer:
May be due to Chlamydia or other reasons.

Detailed Answer:
Hello XXXX,

I had gone through your question and understand your concerns.

Pain in the scrotum can occur in males of any age. Some of the reasons are:

(1) Twisting of a testis. This occurs when the testicle twists spontaneously within the scrotum. Symptoms like: testicular tenderness, swelling and redness. This condition is a medical emergency and requires immediate medical attention.

(2) Inflammation of the epididymis which is as a result of an infection (the most common cause of infection is Escherichia coli). Symptoms like testicular swelling and redness, urinary symptoms like burning during urination.

(3) Inguinal hernia
If you have:
- a bulge in your groin area when you stand upright and cough or
- pain or discomfort in your groin especially when coughing or lifting
it may be an inguinal hernia which occurs as a result of increased pressure within the abdomen ( for example after chronic coughing ) or heavy lifting.

(4) Pain from a disorder in the abdomen like a kidney stone ( pain can radiate into the groin area and cause testicular pain),

(5) Urinary tract infection (UTI) caused by Escherichia coli or Chlamydia. Painful urination is a typical symptom of UTI. Please note that almost half of all men with chlamydia don't notice any symptoms for a long time.

(6) Injury.

I would order for base line:
- urinalysis
- chlamydia test on a urine sample or swab test where doctor will take sample from the urethra
- testicular Ultrasonography
Your local doctors will know about this test.

Before you can see your doctor you can take an over-the-counter pain reliever.

Hope I have answered your question. If you have any further questions I will be happy to help.
