He started with them when he was 24 years old he is now 68
He has had 4 operations on them but they keep coming back

From what you are describing it looks like:
- pressure skin ulcer (decubitus)
- or anal fistula,
which are very common in patients with diabetes. Diabete doesn't let the lesion to be healed normally, causing a prolonged skin lesion.
If you were my patients I would recommend to do the following measures:
- keep his skin dry
- use betadine solution to disinfect his wounds
- apply creams with antibiotics locally
- try to stabilize his diabetes with diet and treatment.
- change his dress every day if needed, since your insurence doesn't cover nurse to do this any more you may lurn to do it by yourself.
