What Causes Upper Abdominal Hernia, Lower Back Pain, Mild Fever And Chills?

Question: My husband is a 47 year old former smoker. Recently, he has been having a myriad of symptoms and we are not sure which area of healthcare providers to pursue. He has an upper abdominal hernia which is new. He also has lower back pain, new onset brown colored urine and low grade fever and chills. He said that the texture of his scrotal skin has changed and that area is sore and itchy. He is saying that his urine stream has many start/stops and sometimes is difficult to urinate despite having a strong urge to go. Do these symptoms all go together and if so, what could it be and which specialty should we see? Thank you.

Dear Patient,

i cannot bring all symptoms together to one condition but that is not unusual since people can have more than one medical problem. I'd like to lay out some possibilities to you according to the symptoms you are desribing:

1.) lower back pain, new onset brown colored urine and low grade fever and chills:
This could be a sign of kidney disease, i would rue that out by doing a urine test, ultrasound of a kidney and blood tests for inflammation and kidney fuction.

2.) urine stream has many start/stops and sometimes is difficult to urinate despite having a strong urge to go:
This can be a problem with prostate and urine bladder, it is important to get an ultrasound of the prostate and bladder to check wether the prostate has grown (benign prostate hypertrophia), which can cause difficulty for urine to pass.

Condition 1 and 2 is best to be checked by a urologist.

3.)upper abdominal/epigastric hernia:
if the hernia causes symptoms like stomach acid reflux, your husband can take acid blocking medication like pantoprazole which will most often give relief. There is also operation techniques to fix those hernias but this option only applies when there is heavy symptoms resistant to the mentioned medication.
This is best to be observed by a gastroenterologist/ internist.
