What Causes Urethral Pain During Urination?

Question: For the past 5-6 days I am having pain in the urethra (the pennis) when I urinate. The pain is not extremely severe, but is definitely a pain.
The pain is most severe when I urinate while I am defecating (bowel movement). For the rest of the day, it hurts most when I am starting the Urine stream and then it gets better, and then it hurts again when I finish urinating and my muscles tighten. The pain goes from the pennis back to the anus.
I did do a prostrate exam and the prostrate is not very enlarged or anything. My PSA is 0.9. I am 53 year old male.

Please help

Thanks for your query ,based on the facts that you have posted it appears that you have pain in urethra while passing urine which radiated to anus .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have painful ejaculation ?
2) Do you have any urethral discharge ?

This is mostly due to Acute Prostatitis and needs to be confirmed .Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to establish diagnosis
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Semen examination and seen culture
4) Swab test and culture of urethral discharge obtained after prostate massage .

In the meanwhile start taking Doxicycline and Diclofenac twice daily .Later on switch on to appropriate antibiotic

Further treatment will depend upon result of these tests and final diagnosis.

Ejaculation is not painful, but for the last week or so, it is dry with very little semen
Also, there is no uretheral discharge

If you see my previous thread, I had a kidney stone of 4mm size. While the thinking was that I had most likely passed it, in reality, I had not.
I passed the stone last evening and right after that, all the pain and symptoms went away magically! It was likely stuck in the bladder and perhaps at the bladder- urethra junction causing me all the pain and discomfort. When I went to urnate last evening, the pain became intense and then the stone popped out with urine.

Now, I need to really figure out how to avoid this going forward. Starting with 3+ liters of water a day.

Fortunately, I was able to catch the stone, so I will be taking it to the lab for analysis and once the results are out, I need to understand the changes in my lifestyle and diet that would help prevent a recurrence.

Thanks for follow up .

It is good that you have passed stone .Obviously it was the stone impacted in urethra giving you the pain .

Consult your Urologist with report of stone analysis ,he would guide you to modify yur diet to prevent recurrence of stone .

Ensure to drink more water so that you will pass more urine .This will help to prevent aggregation of stone particle and prevent stone formation
