Based on the facts that you have posted burning urination that you have could be mostly due infection of Prostate (Prostatitis).
I shall be happy if you can post me more details to help me in making precise comments
1) Do you have painful erection ?
I would suggest you to discuss with your Urologist about having infection of Prostate (Prostatitis) and get following tests done to confirm the diagnosis
1) Ultrasound Scanning of abdomen and Trans Rectal Ultrasound(TRUS)
2) Semen examination and Semen culture.
3) Swab test and culture of Urethral discharge collected after doing Prostate Massage .(This would help to get secretions directly from prostate and help to establish precise diagnosis )
Till then continue antibiotics being prescribed along with anti inflammatory drug like Diclofenac twice daily.

1) - They scanned my bladder when I went to visit, to see if any urine was left/withheld.....came back fine.
2) - There's no blood in semen, and no painful ejaculations (though once recently felt a little sore after)
3) He did check prostate and said it was normal
The NP I first saw thought it was Prostatitis too, but the Bactrim did nothing for it. If the prostate is normal, how could it still be (possibly) prostatitis? Initially, I did have bacteria in urine (was given macrobid) but since then all clean. That's what I don't understand.
Also, around the opening of the meatus, it looks red/irritated. Definitely not normal.
There is entity called Non Bacterial Prostatitis .

As explained earlier Semen culture and secretions collected directly from Prostate that you collect after doing prostate massage can detect organisms causing infection
.There is one entity called Non Bacterial Prostatitis where one has symptoms similar to Prostatitis with no positive evidence of infection of prostate what is called as Non Bacterial Prostatitis .Our aim is to establish diagnosis by doing all these tests so that we can plan future course of action and treatment .

This conditions usually occurs in middle aged males due to abstinence from sexual activities for a prolonged period .Doing prostatic massage or discharging prostatic secretions by masturbation 3-4 times a week does help to resolve the issue ,However it is advisable to do these tests to rule out bacterial infection .
For redness around tip of penis take anti inflammatory drug Diclofenac twice daily .
