What Causes Urinary Retention When Suffering From Diabetes?

Question: good morning sir....
my father is diabetic patient. he was diagnosed to be diabetic around 4 months ago and now his diabetes is under control....but since 2-3 days he is experiencing a new problem of urine retention. the feeling of retention comes only once during the urination in the morning,when he wakes up..he says it feels like around 30% of urine is still remaining and and it is emptied around half an hour after the first one.. so i wanted to know is this a serious problem or is it common with aging(my father is 51 years old). thanks in advance..(also my father had an issue of disc migration around 3 years ago in lumbo sacral section..dont remember the actual disc number in this region)

Hi
The symptoms you gave outlined is what we term obstructive LUTS - lower urinary tract symptoms. He should be evaluated with a few basic investigations and these include an ultrasound examination of prostate and bladder with an estimate of prevoid and post void urine. He should have a test called uroflowmetry done and depending on these we will be in a position to advise you if he needs medication. For example if his flow rate is slow, say, less than 15ml/sec, and he has a residual urine over 100ml, he can benefit from use if medicines belonging to a class known as alpha-blockers like tamsulosin. If ultrasound says the prostate is definitely enlarged, other medicines like dutasteride may be required.
As of now, I would say it is unlikely that he will have any major problem, considering that he is fairly young, 51years and he has diabetes only for past 4-5months.

thanks a lot sir... but the problem is my father has a bad history with medicines...medicines like levofloxacin and orfnof has turned out into serious reactions in the past...so my point is are those medicine classes listed by you safe? means is there any chance of reaction? also if he is diagnosed o have obstructive LUTS or enlarged prostate,can it lead to major complications in the future? also at this point of time should we consult a general physician or a urologist?

As a matter of fact, I am not suggesting that he should take any drugs. He needs investigations, which are themselves fairly simple, and as I said earlier at his age, it might quite turn out that he needs no significant medication.
On the other hand, it would be difficult to withhold medicines in those who need it especially if they help prevent bladder and kidney damage.
It is difficult to predict if he is likely to be allergic to these medication. I have never seen anyone reacting to these drugs yet.

sir can it happen that anyone is suffering from obstructive LUTS or enlarged prostate and he only suffers from urine retention as my father has no other observable symptom such as pain in abdomen or blood in urine etc. thanks

I doubt if he has chronic retention of urine which can exist without any significant bother or symptoms in the patient. But as I said earlier, the chances of such an event will be low.
I would advise that a Urologist will be a better option to consult rather than a physician.
