Question: I am a 52 year old male in good health, i am having extreme difficulty urinating and have some pain in testicle area
1) I do not have a burning sensation while urinating, which leads me to believe no infection or STD. I have no fever either.
2)I do have pain both in the urinary area and with the right testicle
3)I definitely have to strain to urinate most times urinating. Especially when I have alot of urine to release.
4)My urine stream is very poor

Thanks for your query ,based on the facts that you have posted it appears that you have difficulty in passing urine and pain in testicles .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have urinary symptoms like burning urination ,frequency for urination etc?
2) Do you have pain in perineal region?
3) Do you have to strain to pass urine ?
4) How is your urine stream? is it poor,moderate or normal ?

You need to Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.
4) Swab test and culture of urethral discharge obtained after prostate massage .

In the mean while start taking take broad spectrum antibiotics like Cefotaxime along with urinary antiseptic like Nitrofurantoin twice daily and urine alkalizer thrice daily .Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

Ensure to drink more water .To keep your urine dilute This will help to control dysuria.

Further treatment will depend upon result of these tests and final diagnosis.

Poor stream with difficulty in passing urine with straining to pass urine could be mostly due to narrowing of urethra passage (Stricture Urethra) and needs to be confirmed by a radiological test called Retrograde Cystourethrogram .

Get the test done under guidance of a qualified Urologist .

Further treatment will depend upon the findings of this test .You may need to undergo an endoscopic procedure called Optical Internal Urethrotomy .
