Question: Hi, I am 52 and went through the menopause without too much problem until now. I haven't had any children and I went through the menopause around 4 years ago. This year it has been increasingly difficult to have sexual intercouse and have been prescribed vagif em pessarys . I still cannot have sex and may need to use sort of dilator. Can you advise if and what hrt I should consider as my quality of life is much affected. Although I still get the odd flash I can cope with any other menopausal affects. Thanks in advance.

1. After passing menopause, the hormone Estrogen, declines in your body.
2. This leads to vaginal dryness and discomfort on intercourse.
3. While Vagifem is a good medication and a biologic to use, this might not
have solved your problems.
4. Why not try progestins?
5. Since I cannot give you a precription over the internet, the coolest and the best solution is that you visit a doctor, and he or she will most probably put you on this HRT aletrnative, if you are upfront about problems encountered with oestrogen HRT.

Thank you for your advice, so you don't think a combined hrt would work better if my estrogen levels are so low. Can you advise on what dosage is considered a low dose.

1. I seriously do not and would not take a combined HRT if I were you, because we know that though these chemicals masquerade as your bodies hormones (like oestrogen). Your body, in the premenopausal era that you lived in, knew EXACTLY TO A MILLIONTH OF A GRAM how much a "dose" of oestrogen to produce.

2. HRT v/s natural body oestrogen is sort of like comparing primitive stone axes.

3. So what? To calculate a dose depends on many facts about your body that can be known right from simple things like taking a weight and height test (to calculate BMI ratios) to complex things.

4. This is because you deserve the best experience with HRT.

5. If either you could send simple reports, (any can help), then we can determine the exact "dose" of HRT you need.

6. Dear XXXX, remember that a drug in a the right dose is a medicine and in the wrong dose is a poison. This applies to all drugs.
