I am really sorry for what you are going through.
Based on your symptoms and sufferings, I believe the only cause for all your problems is your internal ear infection.
If untreated such internal ear infection can cause complications like vestibular infection (needed fr balance), extension of the infection to the brain causing brain abscess, facial nerve damage etc.
Loss of balance / vertigo is evident , tingling over the face, drooling is indication for facial nerve weakness and tingling in the shoulder and its weakness could be due to brain damage if any.
I strongly recommend you to get completely investigated over this to start treatment before its too late.

Instead of the heart monitor a Carotid artery Doppler would become much useful here. This is a test where in blood supply to the brain is checked whether is sufficient or not. It is in the case of insufficient blood supply you will pass out or have black outs.
