default
Posted on Sat, 29 Aug 2015
Question: Hi, I have the highest possible ANA titer at 1:2560 and no symptoms. And Im 26. Does this absolutely mean something is wrong? (And please don't just tell me to see a specialist?) :) I feel completely fine-- My dr checked my thyroid-- it was normal. She check for RA-- came back in a normal range. But My CRP- C-reactive proteins came back 16.4 "Above High Normal" Related? Thank you!

You need to be in follow up.

Detailed Answer:
Dear Madam,

Thank you for posting your query on HCM

Well very high titres of ANA cannot be ignored. Although you may not be having any symptoms at present, but there is a high probability that you may develop Lupus in future. Have you had further testing like ENA profile, Urine routine microscopy to look for proteinuria, LAC , CPK etc. If not, I suggest you visit a competent rheumatologist and get these done.

ANA positivity can occur well in advance before disease manifests. So this cannot be ignored and such high positivity cannot be false.
If you are married do you have any history of abortions?
Do you have family history of autoimmune disease/lupus?
I would also be interested to know why ANA testing was done in the first place when you don't have any symptoms? what made your doctor request this?

Please let us know all relevant history.

Take care.

Hi Dr. XXXXXXX

Thank you for getting back to me. And, oddly, yes... I had an abortion 8 years ago-- what would this have to do with the ANA? I have never heard of a connection?

I have not had those other tests, though I have no family history of any autoimmune diseases. I first had the test done last fall because I got the flu vaccine (my first one ever) and had a terrible reaction in my muscles-tissue around that area of my left arm. My muscles hurt so badly that I could barely lift my arm to get dressed. The muscle thing lasted for a solid 5 weeks (even after my dr put me on an anti-inflammatory drug for a week). The muscle pain transitioned to a joint pain that spread down the other arm and hand, but was always worse nearest the area I received the vaccination. Finally, after 2 months, all the pain and swelling (because there was visible upper arm swelling) went away, and I've been feeling fine for the past 7 months. But, the ANA came with a 1:2580 in Sept 2014, and we redid it in March 2016 where it was 1:2560.

Have you ever heard of these symptoms in conjunction? Or any idea what all that was triggered by the flu vaccine?

I really appreciate your answers. Thank you so much. Best, YYYY@YYYY

Brief Answer:
Please see an experienced rheumatologist .

Detailed Answer:
Dear Madam,

Flu vaccines can temporarily give ANA positivity but since in your case its continuing it cannot be ignored . SLE is associated with Antiphospholipid syndrome which is associated with recurrent abortions so I asked this question to you and it occurs in young females .

In my opinion you should see a rheumatologist for the same . If you have any queries regarding anything please let me know.
