What Causes Voiding Dysfunction And Blood In Semen After Taking Combodart?

Question: Hi,
I am 62 years old male.
I have had bph for three years and been taking Combodart ( dutasteride/tamsolosin for tw years.
I have no trouble with urinating ie frequent nightly trips, post voidal dribble, straining to pee etc.
I do not fully empty my bladder, especially first pee of the morning.
Never have an urgency to go.
Prior to the Combodat I did have occassional uti's. Since adopting this drug no more uti's.
A week ago I had a pink tinge to my ejaculate, sperm. Yesterday there was a brown tinge to my sperm. I presume this being old blood.
Does this indicate an end to this worrying affair?
My last psa was normal albeit that it was nearly twelve months ago.
Does it indicate something more sinister?

Thanks for your query ,based on the facts that you have posted it appears that you are known to have Enlarged Prostate and have been taking combination of Tomsulosin ans Dutasteride for over last 3 years and have no issues as regards voiding urinary symptoms .

However you have noticed your semen to be brown in colour since very recently.It is due to a small amount of blood being mixed with semen as rightly pointed out by you .

The commonest cause of blood in semen (Hematospermia) is due to infection of prostate .

Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.
4) Semen Examination and semen Culture.

In the mean while take broad spectrum antibiotic like Cefixime along with anti inflammatory drug like Diclofenac twice daily .

Further treatment will depend upon result of these tests and final diagnosis.

Can we discount prostate cancer ?
Why has is have to be an infection. Is it possible that the urethra is being squeezed by the enlarged prostate and rupturing a blood vessel.
when you say infection do you mean uti ?
If there was an infection would I not have blood in my urine ?
If indeed it is an infection is it treatable ?

By infection i mean infection of Prostate (Prostatitis).

Thanks for follow up.

Whenever there is enlargement of prostate there is an element of congestion and infection of prostate (Prostatitis).Occasionally a small tiny vessel on surface of the prostate opens up and causes slight blood which when gets mixed with semen t give it a brownish tinge .

As regards your doubt about Cancer of the prostate it can be confirmed only on digital rectal examination and doing Serum PSA and if it is raised by doing a biopsy of prostate .

The infection of prostate can be treated with appropriate antibiotic aas per culture report.

Hi again,
With prosatitis would you not have other symptoms ?
As the semen a week ago had a pink colour and now brownish tinge (old blood) has this episode passed.
Perhaps a wait and approach would work ?

A couple of weeks ago I had a brownish tinge to my urine.
I had a urine tested for infection which proved negative XXXXXXX
