What Causes Whining, Rubbing Of Nose And Rapid Finger Twitching When Excited?

Question: My wife and I have been caring for our grandson since February. He comes from a troubled home and has been diagnosed as Autistic, with ADHD, Pervasive Defiant Disorder and Math Disorder. From my understanding of a clinical report he has been on a parade of medications since he was 4 years old. He has been on a drug called Invega for several years. 4.5 mg per day at evening. His new psychiatrist says that this the largest dose that she has prescribed for patients. Our grandson has a behavior which consists of whinnying and rubbing his nose with both hands together and fingers twitching rapidly. He does this when he is excited about something he is doing. He also has trouble sleeping through the night. He can get to sleep, but every night he gets up and finishes sleeping either in my bed or my wife's bed. I am wondering if this can be attributed to medications and is related to tardive dyskinesiai

He is having symptoms of autism, ADHD, pervasive development disorder and mathematics learning disorder. He is on medicines since he was 4 years old. He has been on Paliperidone 4.5 mg per day for these disorders. Now he has developed habit of rubbing nose and have finger twitching. He has problem of sleep also.

Most likely the finger twitching are seen due to tics. Tics are causing discomfort in him which compels him to rub his nose. Tics can be due to ADD pathophysiology. In a lot of individuals of ADHD tics are common finding.

Other possibility is drug Invega (paliperidone). Invega can potentiate tics in some individuals. This is surely not tardive dyskinesia. Consult his psychiatrist for any change in drugs or dose adjustments. He is on high doses so any change will help him. Clonidine is a good alternative for him. The drug will decrease the abnormal behaviours and will increase desired behaviour.

Discuss with his psychiatrist for detailed management.
