Posted on Fri, 11 Jul 2014
Question: I have a white coating on my tongue for the past 8-10 mo. Have been to several Doctors, and thet say it is normal. The tongue also burn like it has been burnt by coffe a day before, but it's all the time. I have lumps on the sides of it too. I have never had this in 70 years of my life. How can it be normal? I am really concerned over it. They have taken no lab test of it. At present a doctor has me on Dilantin. I can not see how this could do ant thing to help.

Brief Answer:
frictional keratosis, leukoplakia, candidiasis

Detailed Answer:
Hi,

Thanks for your query, I have gone through your query.

The white coat on the tongue could be because of the chronic irritation by the sharp teeth resulting in frictional keratosis. Or it can be leukoplakia or lichen planus or it can be fungal infection.

If it is a frictional keratosis then we have to remove the cause like rounding off the sharp cuspal edges.
If it is leukoplakia, you have to stop the habit of smoking(if you have) and use antioxidants like capsule oxyace once daily for 30 days.
If it is lichen planus then you can use topical steroids like triamcinolone acetonide (0.1%) 3-4times daily for 7 days.

It would be extremely difficult to recognize which of these three is the most likely without physical examination. Please consult a oral physician to rule out all these things and treat accordingly. we can also do vital tissue staining to rule out the white lesions. A biopsy may sometimes be necessary.

I hope my answer will help you. take care.
if any queries are there reply me back.
