It is difficult to make a determination without seeing the lesion. I would recommend that you see a dermatologist or local gynecologist.
In addition annual mammography is important as you get older because the risk of breast cancer increases with age. Nonetheless a little yellowish discharge forming a scab on the nipple does not sound like the presentation of a malignancy to me. but of course it is better to be safe than sorry. Almost 90% of stage 1 and 2 breast cancers are entirely treatable so early detection is imperative.
So to sum up it's important to rule out the worst thing but if I had to bet I would say this is probably a simple dermatologic issue and not representative of a malignancy. Go get it checked.
