What Could Cause Difficulty In Passing Stool Initially But Easily Passes Later On?

Question: Hello Doctor
My wife has unique complain of constipation where she complained that it is difficult to pass stool in the beginning but later it passes easily. What kind of constipation is it?
I am advising her to take plenty and water and other fibre diet. Do you suggest to take any homeopathic medicine to treat it?

Hello,

I'm of the following opinion:

1. If her diet/lifestyle is good and does not suffer from any disease or is not taking any medicine(s) it could be termed as functional or primary constipation which occurs in women, however, before jumping to this conclusion one must exclude the following reasons:

i. Diet: is she taking too much of cold-dry-astringent-slimy food?
ii. Lifestyle: physical inactivity?
iii. Medicine(s): taking iron-calcium-lead-katechu-nutmeg supplements or psychiatric medicine(s)?
iv. Disease: underactive thyroid [hypothyroidism], Piles, neurological disorder, or Grahani [Irritable bowel Syndrome], anemia,weakness? [any investigation / examination done]
v. Psychological: depression, personality dysfunction?

2. Do let me know about the above questionnaire, till then

* Regular bowel habits with attempted bowel movements 15-20 minutes after breakfast when gastro colic reflex is strongest.
* High fiber diet.
* Adequate fluid intake.
* Abhyarishta/Panchskara churan/ Tab/Cap Herbolax, Nux vomica, Sulphur,, Bryonia,Lycopodium, Graphites are prescription medicines which are not habit forming and can be used safely [in recommended and prescribed dosage]

3. Diet:

Take: old brown rice, pigeon pea, green vegetables, papaya, carrot, radish, cucumber, cabbage, bottle gourd, mango, grapes, musk melon, raisins, dates, oranges, pears, guava, honey, milk, butter milk

Avoid: newly harvested rice, black gram, peas, banana, potato, spices, fast food, late night awakening, suppression of natural urges, physical inactivity.

. purgation with olive oil is free from side effects.
. early morning [empty stomach] take either

- eat 10 pistachio + 4 raisins
- 100 gm tomato juice
- drink water after coming out of bed

She normally avoids it,

Busy with households work. No dedicated physical activity.

Yes, she is anaemic and taking iron and vitamin C and multi-Vitamin as suggested by doctor.
iv. Disease: underactive thyroid [hypothyroidism], Piles, neurological disorder, or Grahani [Irritable bowel Syndrome], anemia,weakness? {any investigation/ examination done]
Anemia. Other results were negative.
v. Psychological: depression,personality dysfunction?

Hello,

1. As she is taking iron/calcium supplements both of them causes constipation in humans and when taking both simultaneously, they compete each other for absorption, so following points must be kept in mind:

i. The body has easier time digesting iron/calcium found from natural sources [so go for the same] OR start with half of the recommended dosage to give time for the digestive system to adapt

ii. With Iron take Vitamin C [citrus fruits] and with Calcium take Vitamin D [D3] for better absorption of the two
PS. consider taking a magnesium supplement [since it is needed to fully absorb and utilize calcium]

iii. With these [as said earlier]: fiber supplement/adequate hydration/exercise/ laxative is important [make sure to rotate laxatives to prevent habitual formation]
PS. make sure to take at least 1.5 L of water/orange juice with iron pill since it will provide necessary hydration to the intestine [to make soft stool] and vitamin-C will help absorption of iron

iv. Prescription medicine(s) without adverse side effect are: Mannol Malt, Panchskar Churan, Lauha Assav, Praval Pishti.
