Brief Answer:
Occipital neuralgia to consider.

Detailed Answer:
Hello and thanks for using HealthcareMagic.

I have read your question and understand your question.

In my opinion, occipital neuralgia, a condition related to compression of upper cervical nerves that supply back and top of the head.

Arthritis of the cervical spine may be one of the conditions that may cause such symptoms.

I suggest you discuss with your Doctor about this condition and further tests such upper cervical spinal x-rays and MRI.

Since you had physical therapy and painkillers used, epidural injections could be considered as the next treatment modality.

Hope you found the answer helpful.

Take care.
