I can understand your concern. You are having head/neck/shoulder pain, congestion, fatigue, clogged ears, nasal drip, dizziness. Usually such kind of symptoms are seen in
- Air sinus infection
- Upper respiratory tract infection like infection in nose/pharynx/ear
- Chronic fatigue syndrome
Now to differentiate between these, I need more information about you like
- What is the total duration of symptoms?
- Is there any fever?
- Do you have history of nasal blockade/heaviness of head especially on forward bending?
- Is there any history of allergy?

I've had these symptoms since last Monday. I was on a 9.5 hour flight from Japan to Los Angeles and the symptoms showed the following day.
I do not have any fever now but following the last Monday when my symptoms arrived I did have the cold sweats where I was getting cold and hot and cold and hot.
I do not have a history of nasal blockade or a history of allergies.
My under chin does feel a bit swollen and when I bend my neck it feels swollen under my chin. Overall lack of energy from the headache.
Sinusitis or URI most probable diagnosis

Thanks for providing me the answers. Your history is suggestive of upper respiratory infection or sinusitis. The lack of energy, fever in the beginning and heaviness all pointing towards URI.
Treatment of both diseases are same. Kindly follow these steps to get relief
- Inhale steam 4-6 times daily
- Take rest. Avoid doing work which require focus or exertion. You will recover fast during rest.
- Eat warm/hot food and drink hot drinks like coffee/tea/milk. Avoid cold food/drink like ice-cream/milkshakes.
- Use warm nasal saline drops. It will help in opening the sinuses and relieve the nasal drip.
- For congestion and headache, take combination of Paracetamol+Cetrizine+Xylometazoline after prescription. Take it thrice a day.
- If your nasal discharge is yellow/greenish, it means it is bacterial infection. In such case, you need antibiotic course like Coamoxyclav or Azithromycin therapy. Start any of these medication after prescription.
- Preferably sit in warm environment. Use heater/blower for it.
If still no relief, you should consult ENT specialist. He or she might advise you Xray of paranasal sinuses to know about chronic sinusitis.
