Question: Dear doctor,
i had exposure with a call gal of unknow status of hiv
she gave me unprotected blowjob for 10 sec i m uncircumised guy, the she stopped, then we had naked forttage for 3 to 4 Min were my bare penis came in contact with her vagina and after 2 min their was vaginal secetion on her vagina (fresh fluid) came in contact with my penis tip many time i worried for tht i m uncircumised guy

i did some test

13 day
1) Hiv 1 RNA qualitative (combas method detection limit 20 copies/ml)
2) Hiv 2 RNA qualitative (Real Time PCR method detection limit 900 copies/ml)
3) hiv duo test (ELFA method) 0.05 Negative

29 day
1)Hiv Duo test(Cmia method) 0.08 non reactive

do i need more testing or else i can conclude myself

Thanks for posting your query.
First of all, the girl, you met with, is not necessarily a HIV positive individual.
Oral sex carries a low risk for HIV infection. Frottage has negligible risk for HIV. Contact with Vaginal secretion without any penetration (unlikely to have abrasions) has also negligible risk.
HIV PCR tests can detect the HIV infection (if any), as early as 7 days after the exposure.
You had PCR test for both HIV 1 and 2 infection separately and also 4th generation test after 28 days.
These tests ruled out the possibility of both HIV 1 and 2 infection. You need no further test for HIV.

Thank you doc for your prompt reply, i heard many people will not test at positive at 28 so who are those case who dont have P24 detectable at 28 days
means does pep effects or any other things effects the window period

There is nothing like that the P24 antigen test is not reliable by 28 days after the exposure in certain individuals. Only the commonly performed antibody detecting tests like ELISA,Rapid, Western blot test are not reliable till the completion of window period (6-8 weeks , sometimes up to 12 weeks). These antibody detection tests may be delayed further up to 6 months in situations like the individual is on immunosuppressive drugs or have taken PEP with ART. But not with P24 antigen test.
You need not bother about these. You are free from HIV 1 and 2 infection.

Thank you doc you hav put ma anxiety level to ground, just last question even if she was positive ma test are reliable now i guess.
Even if my penis tip urethra came in contact with the fresh vaginal fluid then also it is consider negligible risk without penetration, i didnt have any abrasion on penis at tht time its 7 week no symptoms as such

hello sir,
warm good evening,
i had risk exposure for hiv i recived oral sex from call girl and Naked Forttage where Bare penis came in contact with vagina and its vaginal fluid touched my tip of penis urethra
some test done after exposure

13 day
1) Hiv 1 Rna Qualitative (cobas method) 20 copies detection
2) hiv 2 Rna RT PCR method
3) hiv duo (vidas machine)
all negative

day 19
cbc wbc was 11500 (4500 t0 11000) may be duo to bacterial infection
neutrophil was 93 (40 t0 80)
lymphocytes 13 (20 t0 40)

day 23 cbc
came back in normal

day 29
Hiv duo (abbott CMIA) 0.17 non reactive

day 40
cd4 and cd8
where cd4 was 420 (450 to 1600)
and cd8 was 498 (280 to 1000)

ratio is 0.85 of cd4/cd8 (0.5 to 3)

my question is
1) can lab error occure in hiv duo test on day 29 which i did

2) what happens to cd8 cell in recent hiv does it goes up or down and after how many days this cd8 cell change after infection

3) and how much cd4 cell decrease in recent infection till 40 day will it be more down than 400

1.Technical error in DUO test is quite unlikely in a bigger lab with systematic protocols for any test.
2. CD8 count in HIV infection is not important in HIV infection. No specific response is expected.
3. It depends upon the individual's body response to HIV infection and the activity and virulence of the virus.
But your activities carry a low risk for HIV and the test reports were all indicating that you are free from HIV.

will wbc increase in hiv from week 2 to week 6 i had 5 cbc and i saw my wbc was increasing continuesly from 8600 to 11500 and at 6 week its was 10 500 always its stayed at higher end and with increasing wbc i have high neutrophils 75 to 83 percent and my lymphocytes was 1300 at 2 and half week after tht it increased to 2000 the next week what my be the cause if i had hiv infection my wbc would be low or high and lymphocytes low and what happen to neutrophil

sir i know we cant analysis from cbc just a general question what happens to wbc will it go down or rise just wanted to know and thanks alot for your help pls just let me know what happens to wbc in acute hiv so that i can test my self at 3 month mark with antibody just for peace of mind

Usually in viral diseases WBC raise is not a routine event. In early HIV disease, there will not be any specific changes in the WBC count is observed. Thank You doctor Now i feel confident thanks for your support sir, Actually i was worried, on day 20 i had my Cbc my wbc was 11500 and Neutrophils was 83 percent and lymphocytes was 13 percent and after 2 days i had another CBC Where My WBC was 10500 and Neutrophils 76 percent and lymphocytes 19 percent,

and till 6 weeks my lymphocytes increased only and my platelet also

My last question Hope u will help me

1)If I was infected then my lymphocytes wont have increase that soon or it can increase without medication also

2) then my 13 day rna hiv test would be positive

3)Hiv virus also attack platelet in acute phase or not, my platelet was increasing only till 6 weeks

HIV has no direct action or influence over platelets. The increase in platelet count in early HIV infection can not be attributed to HIV infection.
Increase in WBC count and especially Neutrophils indicates the possibility of some acute bacterial infection rather than HIV infection.
