Posted on Tue, 27 Sep 2016
Question: HIV Specialist Only Please.

I would like some help on following reports as CBC and CD4 reports are not looking great. I had riskiest exposure(s), ARS symptoms that came and gone leaving my body in bad shape. I do have some symptoms even now.

I know that with initial infection cd4 drops drastically and i believe this is what's happening....

Can you please educate me on this one ? Should I be worried ?

CBC Report

WBC @ 2 month 9100, @ 3 month 8400, @ 4 month 7600 ---> gradual decline
Platelets @ 2 months 274000 ,@ 3 month 257000 @ 4 months 248000 ---> gradual decline

CD4/8 Report

CD4 absolute @ 2 month 1622 ,@ 4 month 851 --> almost 50% decrease
CD4% @ 2 month 44% ,@ 4 month 41%

CD8 absolute @ 2 month 1060 ,@ 4 month 623 --> almost 50% decrease
CD8% @ 2 month 30% ,@ 4 month 29%

CD3 absolute @ 2 month 2704, @ 4 month 1482 --> almost 50% decrease
CD3% @ 2 month 74% ,@ 4 month 72%

CD4/8 ratio @ 2 month 1.53 , @ 4 month 1.36

Brief Answer:
No need to worry.

Detailed Answer:
Dear XXXXXXX

Welcome to HCM.

You said that you had risky exposure of HIV then the drop in WBC, CD4, CD8 and CD3 count is definitely due to stress or anxiety nothing else because all the counts are still looking very good and do not indicate towards any immune deficiency so you need not to worry at all. Be relaxed and move ahead in life happily.

Thank You

i have read and saw it on several site including cdc that in initial infection rna increases so much and cd4 reduces so much as well....so in theory i jad cd4 of 1600 2 months ago and now 850....even though % ratio remains same overall absolutes counts had cutdown in 50% for cd4,cd3,cd8.

i had the same anxiety and stress even when i took first test.

what could be the reason for this drop ither then hiv?

Brief Answer:
Stress or anxiety is the only reason.

Detailed Answer:
Dear XXXXXXX

Welcome back and thanks for follow-up.

You are absolutely right that in initial infection CD4 count falls dramatically but not in your case as the count is satisfactorily good enough and the reason for drop is stress or anxiety not HIV so calm down, forger about the episode and move on in life.

my main concern is i had cd4 of 1600 at 2 month of exposure and now 851 at 4 months.....4 month duo is negative but i know that,

3% of the ppl develop antibodies between 3 to 6 month...

1)coinfection with hep c
2) immuno deficient,chemo therapy,etc...

i understand some variation but 50% reduction whike exposure,symptoms....i cant understand real reason except hiv

Brief Answer:
It can be due to stress also.

Detailed Answer:
Dear XXXXXXX

Welcome to HCM once again.

HIV Duos test at 4 months is negative which is not only reliable but also conclusive enough. I agree to you that some people will develop antibody during 3 to 6 months period but only those with Hepatitis C co-infection or immune deficiency no one else.

The reason for drastic fall in CD4 count is stress or anxiety in my opinion not HIV so no need to think too much about HIV because you are not at risk in any way. Consider counselling services if needed. Wishing you very happy and healthy life ahead.

does having cmv mean having hiv?

is cmv curable?
doctor
Answered by Dr. Sunil Jangid (8 hours later)
Brief Answer:
It is nothing like that.

Detailed Answer:
Dear XXXXXXX

It is true that CMV infection is more common in HIV infected patients but it doesn't mean CMV is always associated with HIV and it is curable also, so avoid these kind of thoughts, just calm down and forget the episode as you are safe and risk free.

Brief Answer:
Not HIV.

Detailed Answer:
Dear XXXXXXX

You had risky exposure but all your test results are negative for HIV at 4 months. The drop in WBC and CD4 count is most likely due to stress not HIV. Get relaxed, allay the fear or anxiety and move ahead in life. Wishing you very happy and healthy life.

the one thing that i dont get is that i had a cd4 of 1600 2 months after my exposure even though i was in so much stress and anxiety. then between 2 month and 4 month, i had very good diet, some exercise, no stress and anxiety as hiv test was negative at 2 months....

i had exposure, symptoms for some time, high cd4 initially, good playelets. white blood cells..

now low cd4,cd8,cd3,platelets,wbc..

is there anything other then hiv that could cause DROP in all these?
doctor
Answered by Dr. Sunil Jangid (8 minutes later)
Brief Answer:
It can be due to some other infection.

Detailed Answer:
Dear XXXXXXX

Welcome back and thanks for follow-up.

The drop in WBC and platelet count might be due to some other viral or bacterial infection but fall in CD4, CD8, CD3 is most likely due to stress or tension nothing else in my opinion. Consult your physician, go for some investigations to isolate the cause and follow the treatment regime. Be relaxed and move ahead in life.
