Question: As per your SMS yesterday I want to say thank you to you for remembered me.

Sir till now I'm not well the conditions is going more critical these days .
Dr.
I'm summiting more pathlab reports to your attention.

All day 24hours I have pin point pain nearby heart area & arm+shoulder.
It is increased after taking sum meel & headache too.
I'have no family it is difficult to survive me due to illness Please diagnose and treatment
to me. Thanks

Dr,
Sir I have pin point pain nearby heart area & arm+shoulder.
It is increased after eating sum meal.

the pain is surrounding heart side arms & shoulder area & neck, headache too.
the symptom is increased after 20 dec 2015.

my breathe is broken like and giddy dizzy at sleep time.

before 20/ in dec 2015 one day in early morning i was in sleep suddenly i feel electric shock like
and feel a hazardous head to tail quake in my body and hear a HUNTER SOUND LIKE.
after that day to till the condition is gone more critical.

i already submitted all my path reports and earlier details and try to second time SIR.

Please look in to my matter sir.

Thanks for your response.
I have read your message.
I have read your reports.
I have considered the clinical details in your case.
Your reports are clear about 2 things-
1. you don't have a heart problem
2. you do have a kidney problem.
I think there are a few tests you still need to get done.
In view of your symptoms- pain after eating food- I will need to consider pancreatitis and severe gastritis as a cause. I therefore suggest testing Amylase Lipase and an ultrasound of the abdomen. Eventually, you will also need a endoscopy if the above tests are normal.
In view of the kidney disease, I also suggest testing Fasting and PP blood sugars with HbA1c test.
In view of the breath sounds I will suggest a Chest X ray and a Pulmonary function test.
And yes, you need to meet the nephrologist for your kidney problem as suggested by the doctors at Safdarjung Hospital.
