What Do My Lab Test Reports Indicate?

Question: Hello Doctor, I am XXXXXXX I was facing a throat problem and i wanted to brief you the total case.

" I was in Germany begining of Nov 2015 and had 3 sexual encounters in 2 days with Commercial Sex Worker. The sex was unprotected Oral sex full fledged (giving and receiving both) and protected Vaginal intercourse. This was the first time I had such encounter and was really guilty for the incident happened. Actually I was in a drunken and heavily smoking during that time.

After coming back I was really stressed and had oral thrush, and mild sore throat. I took antibiotics for 3 days and was ok. After a week i still have mild irritating throat , mild pain on the lymph nodes.

Finally after 3 weeks of risk exposure i got tested for following:

1. HIV PCR DNA Qualitative : Result NEGATIVE

2. HIV Combo Test (including p24 Antigen) : Result NEGATIVE

3. HIV Antibodies - ELISA (Test strategy -III) - Result NEGATIVE

Also I had also tested for STD (Syphilis, Herpes) which was also Negative.

I also met a doctor and he suggested antibiotics for a week for throat infection. I used the antibiotics but after a week . Also sometimes I still have the mild throat pain ,intermittent pain like squeezing.

Also I had mild pain under the jaws (tonsiltis).

I was really getting worried ,as despite using antibiotics I was still having this irritating throat. Also I developed a rash on the shoulder on 38th day and it has vanished in a day.

I met the doctor again and again he gave antibiotics for a week (Cefpodoxime proxetil) for throat infection. I completed the course of Antibiotics, however I still had very mild throat, sometime on the left side of the throat and right side of throat but rarely on middle of throat and intermittent squeezing pain. however it was only mild and irritating.

So I got tested again as below:

On 41st day HIV Elisa (Test strategy-III) : Negative

On 44th day HIV PCR -RNA- Quantitative- Negative

On 45th day HIV Combo Test (including P24 Antigen) - Negative.

On 53rd day got tested for PCR qualitative test for Gonnoreah and Chlyamdia: Negative

On 56th day HIV Combo Test (including P24 Antigen): Negative

I ve attached all the reports for your reference except the PCR DNA-qualitative report, however it was negative too and was tested at 3 weeks.

Please advise on the following:

Now it is 70 days of exposure again i had mild throat pain, tender and painful lymph nodes and a small rash on my neck area, also had mild muscle pain.

I did gargling for 3 days with salt and used panadol tabs for 2 days. The pain has gone and throat pain is reduced drastically and i stopped gargling as well.

After a day a very mild throat irritating persists. Do I need to start gargling again. Also I was using fucidin for the rash on the neck.

The throat is scratchy and irritating, but no problem while eating and having fluids. Also had icthy ear on both the years.

But I wonder 15 days back I had antibiotics for 2 weeks, i dont know why the throat irritation occurred again.


The doctors said that the HIV Test after 3 weeks and 6 weeks and 8 weeks are negative (both DNA, RNA, Antibodies , P24 Antigen) and are conclusive .

Are these considered conclusive? or do i need t wait for 3 months as per CDC guidelines. I am not having sex with my wife till my doubts are cleared.

Please advise your opinion whether HIV tests are conclusive or still i need to test, as I am planing for kids.

Can you please advise on the medication for irritating throat, why still it persists.

I will try to satisfy you and bring you out of your tension.
Read all your history, about all tests in detail.
My impression is that,
1, Three consecutive STD tests are negative,
2, PCR qualitative test is negative,
It is confirmed that you are free from any type of STD including HIV
Now a about your problem persisted might be due to,
1, as you are given different type of higher antibiotic medicines for long time might disturbed normal intestinal flora might lead to Thrush infection in your mouth giving irritation in throat.
2, Another possibility of having Acid reflux giving rise this problem.
3, you might be developed Anxiety neurosis due to false fear of having STD precipitating your problem.
Now According to my opinion,
Go for treatment as under,
1, Take Sporolac or Lacrtobacillus capsules twice a day for one week.
2, Take Pentaprozole or Omiprazole twice a day for 1-2 weeks.
3, Do Listerene or Feel fresh gargles.
4, Take B.complex capsules.

Thank you for your reply.

So you are sure that I am free from HIV????? are these tests conclusive.

Do i need to test again???

Also I have a gastric problem earlier and used medication for the same a month back. again i started using pantaprozole twice a day.

You suggested following medication

1. Take Sporolac or Lacrtobacillus capsules twice a day for one week.
2, Take Pentaprozole or Omiprazole twice a day for 1-2 weeks.
3, Do Listerene or Feel fresh gargles.
4, Take B.complex capsules.

Can you advise on the sporolac, what it is ? and B-complex for how many days.

Also under the tongue i have mild ulcer pain, will using b-complex solve the purpose.

Please advise

No need of test doing again, all test are conclusive, take Sporaolac.

All tests done are negative confirms that you are not having any type of STD.
Now there is no need to test again.
Sporolac is a probiotic medicine and helps correction of disturbed normal intestinal flora and bringing down to normal leading to cure of thrush infection.
As you are having ulcer under your tongue confirms that there is thrush infection.
Taking sporolac capsules and B.complex for 10- 12 days will give you relief from your all symptoms including ulcer under the tongue.
Take curd, buttermilk good amount.

Thank you clarifying regarding STD, can you please check and confirm regarding HIV test reports , are these conclusive @ 6 weeks and 8 weeks of exposure.

Also as advised I will take the medication . one more thing the throat is mild but some times on the right side and some times on the left side.

Mild ulcer and the lips are to totally dry.

These symptoms are also due to acid reflux.

There are some limitations in tests even with latest technology

Thanks for asking clarification.
In most cases after exposure of 30 days gives conclusive results but in some case it takes some time to develop antibodies and within 60 days it gives accurate result.
So within this time accuracy will be more than 95 %.
Due to modern latest laboratory facility chance of false negative results is rare.
As you are gone for all latest HIV tests like Eliesa,DNA and PCR qualitative tests after 5-6 months are negative so we have to rely upon these tests results.
But there is always some limitation about false negative or false positve result even in USA with latest technology available.
So my advice is to continue the treatment as I mentioned.
Feeling of this irritation at different site in throat can be due to acid reflex.

Dear Doctor,

Just a small correction, the tests were not done at 5-6 months as mentioned by you.

I ve done the HIV 4th generation test at 3 weeks, 6 weeks and 8 weeks of exposure.

As advised i have been using Pantaprozole, B-complex, Actilac (alternate for Sporolac) since 3 days.

But I still have the throat irritation and mild itchy irritation on the ear.

I am really getting worried and tensed.

Are these HIV tests conclusive at the said period????? please advise doctor ( i had protected sex and unprotected oral sex). I got tested for syphilis, herpes at 3 weeks of exposure and Gonnoreah and Chlyamdia at 6 weeks of exposure.

Do i need to test for Hepatitis B. C ad well? Also do I need to get tested for HIV post 12 weeks?

Please advise doctor.

You may go for HIV testing now to get conclusive resilt.

Thanks for clarification.
Now as more than 60 days passed after exposure you may go for HIV test as now it might give conclusive result.
As I wrote to you now antibodies are developed so test will be accurate and sensible.
I do not see any necessity for going for Hepatitis B or C. tests.
Continue with same medicine.

Dear Sir,

I want to ask you a question regarding STD.

I got tested for Syphilis, Herpes, Chlymadia at 3 weeks, but i was under antibiotic for 3days intially and these tests were done after 2weeks of having antibiotics

Apparently after 2 weeks of Cefpodoxime and with a gap of a week i got tested for chlyamdia and gonnoreah PCR Tests.

Can you please clarify whether these tests can be relied as I was having antibiotics.

Antibiotic treatment may affect the tests.

Thanks for query.
As you were given antibiotic Cephodexine medicine for the treatment of Chlymadia, herpes syphilis etc prior to tests of these organisms before 2 weeks might kill the bacteria and so tests for these STD might give negative result.
Therefore it is better to discuss this problem with your treating doctor and take guidance from him.
After treatment if there is no bacteria found, report might be non-conclusive.

I got tested for all STD and HIV again for my peace of mind and am also planning for kids. (please suggest whether i took the correct tests for Hepatitis B and C, I am attaching all the reports)

After 97 days of risk exposure (closed to 14 weeks) i finally got tested for peace of mind .

HIV 1 @ 2 CMIA : Negative
Hepatitis B IGM Antibodies : Negative
Hepatitis C, HCV : Negative
HSV 1&2 IGM : Negative , but IGG shows positive
VDRL : Negative.
TPHA : Negative
Gonnoreah, Chlyamdia : Negative
Culture throat Swap : no pathogens found

Still very mild throat irritation persists,

Please find the attached reports and give your opinion . Now 100% I am free from HIV and STD.

I can have unprotected sex with my wife and plan for kids.

Also please check SWAB culture test, i would need your advise .

Understand you concern.
HSV lab report IGG is positive confirms that you had herpes simplex viral infection in the past.
So there are all chances of transmitting infection through sex.
But accuracy of test is many a time is doubtful so you may not actually assaying your risk.
So my humble request to you that consult specialist for this result, its efficacy and possibility of having false positive IGG test result.,
Your all other tests are negative so nothing to worry about HIV, Aids, hepatitis B and C.
