Question: Report evaluation required
I am getting tired and feeling lazy always. Cant get up in the morning even after continuing medication for 3 months. Got an injury last month and it took long time to heal than normal. Can these effects be healed? or is this something that I need to live with rest of my life? However I definitely felt my situation is better after medication. Kindly let me know if the thyroid intake need to be increased. Also I am getting continuous exposure to sunlight for the lack of Vitamin D.

Detailed Answer:
Hi

I had a look at your reports and here are my observations

1. Your Tesosterone level is low, you need evaluation and possible treatment for this. Low testosterone is often associated with feeling lazy and lack of energy. This is infact more likely to be the cause of your symptoms rather than the thyroid problem.

2. Your vitamin D is pretty low, you need to start vitamin supplementations (Calcirol XXXXXXX 60,000 units once a week for 8 weeks) - please ask your family doctor to prescribe you the same

3. You are Pre-diabetic and your HbA1c report is in prediabetic range.

4. You triglyceride is elevated, which may be associated with obesity, lack of exercise and/or excessive alcohol intake.
5. Is the current TSH report before the medication or after the medication ?

I would adivce you to get done the following investigations in the morning between 8-9 am (fasting)

1. Repeat serum total testosterone
2. Serum LH
3. SHBG (sex hormone binding globulin)
4. Serum Albumin
5. Anti-TPO antibody
6. Serum prolactin



I would suggest the following treatment

1. Calcirol XXXXXXX Calcirol XXXXXXX 60,000 units once a week for 8 weeks then once a month for 12 months
2. Continue your thyroid medication -repeat free T4 and TSH after 6 weeks
3. Exercise 60 min a day
4. Diet control - 1500-1800 Kcal of diabetic diet
5. avoid alcohol
6. repeat HbA1c and FBS after 3 months
