Question: I am a sixty year old male. I recently had my blood work done and found out that my Sexual Binding Globulin Hormone level is at 109! What can I do to reduce or lower this number. Thank you!

SHBG levels increase with hyperthyroidism, cirrhosis, anorexia nervosa, and certain drugs. Long-term calorie restriction of more than 50 percent increases SHB too, while lowering free and total testosterone.
So you should try to find the cause why is high and than treat it.

Thanks for your reply. In looking at what you listed above none of those things apply to my physical condition or lifestyle. Are there any natural or homeopathic products that can possibly lower by SHBG levels.

Welcome back,

If you have excluded hyperthyroidism or corticosteroid treatment, as the most common pathologies that raise the SHBG, then a natural increase of the age is considered.

In elderly male SHBG increases and at the same time that total testosterone levels begin to decrease.

There are some ways to decrease the SHBG;

-Vit D supplements, if you want to increase testosterone (both total and free), and reduce SHBG count, start supplementing with high quality vitamin D3 supplement , be out in the sun, and eat plenty of fish.

-Magnesium and zinc supplements make testosterone more bio-available via decreasing SHBG.

- 6 mg’s of boron glycinate supplement daily for 60 days causes 29,5% increase in free testosterone, which was again caused by a significant drop in SHBG count.

-Of course the most efficient is Testosterone treatment. Testosterone in itself will reduce SHBG count. It’s not exactly clear why, but men with higher testosterone levels usually have lower SHBG levels.

Dear Dr.Guni,

Thank you for the supplement information on decreasing my SHBG and increasing my testosterone levels. Any thoughts on the supplement DHEA in lower SHBG and increasing testosterone. I have also been told that lowering my Estrogen levels would also benefit me. Do you know of any supplements that can be used.

You are right, DHEA treatment at your age has many good effects; raising the testosterone, improving the psychological status, lipid profile, lowers the cardiovascular risk, the osteoporosis, improves insulin resistance ect.
There are also some natural ways that block the estrogen effect;
-losing weight (or, more importantly, body fat)
-reducing your alcohol intake
-adding cruciferous vegetables (like broccoli) to your diet.
Anyway, testosterone therapy is the most efficient therapy in raising testosterone at your age.

Natural anti estrogen supplements;
Especially with regard to obese men, the best form of natural estrogen blockers can be consumed in the form of bee pollen which is yet to turn into honey.
Other effective natural anti estrogen supplements are the ones that comprise of epimedium and horny goat weed. Citrus fruits and other fruits like pears, grapes, pineapples, figs, berries and melons also serve as natural estrogen blockers.

Coming to vegetables, you should have lots of fatty fish (omega-3 fats) , nettle root, onions, cauliflower, corns, green beans, radishes, squashes, brussels sprouts, bok choy, cabbage, chard, arugula, collard green , XXXXXXX and rutabagas to maintain the right levels of estrogen in your body.

Would high SBGH levels cause me to feel tired and fatigued during the day? Also could high SBGH cause me to lose height because of low testosterone levels?

Yes, the symptoms may be due to low testosterone;

Hello again,

As you may know, after the 30-ies, the testosterone level starts to decrease with 1% per year.
So at 60-ies, a low testosterone level is expected, mainly due to testicular production failure.

The symptoms you reported correlate with the side effects of low testosterone and it is to be confirmed by checking total testosterone and SHBG, calculating than the free testosterone or checking directly free testosterone level.

If free testosterone results to be low, you should discuss with your treating physician to start testosterone therapy.

The supplements that help to decrease SHBG or estrogens may be helpful but not always efficient.
Especially if the free testosterone level is too low.

Dr.Guni, I wanted to share my testosterone level with you and get your opinion.
My overall testosterone is 802 and my free testosterone is 9.4. Any thoughts? Also do you like bioidentical hormone treatment. What are your thoughts on bioidentical creams ?

Hi again,

The level of total testosterone is satisfactory and free testosterone is at the lower ranges ( expected at your age).

At this point the decision to take supplements that decrease SHBG is right.
I would not suggest starting testosterone therapy for the moment (especially if the symptoms of low testosterone are absent).

According to the Food and Drug Administration (FDA) and several medical specialty groups, the hormones marketed as "bioidentical" and "natural" aren't safer than hormones used in traditional hormone therapy, and there's no evidence they're any safer.

So, I would not recommend using them.
