Please find the attached reports for the recent tests I had conducted.
1. HB 1C
2. Lipid profile
3. Micro album minuria
4. Creatinine
Present Complaint:
1. Glucose level on fasting: 203
2. Glucose level after breakfast: 221
3. Occasional skin rashes and diarrhea
Present Medication: XXXXXXX XXXXXXX XXXXXXX nashini vati
Age: 66 Years

I am Dr Mittal.
I have read your message and reports.
Follows are my observations and suggestions.

1. SPOT URINE MICROALBUMIN 45.7. This is high.
BLOOD (RED BLOOD CELLS 1 - 2 ) DETECTED (TRACE) IN URINE.
It suggests that you are starting to develop kidney disease. Possibly due to diabetes. You need to control your sugars well to stop the progress of the disease. Consult a nephrologist in your area as soon as possible.
Avoid painkillers and antibiotics without the suggestion of a nephrologist.
I would also suggest an eye check up for retinopathy.

2. GLYCOSYLATED HEMOGLOBIN (HBA1C) 9.3.
GLUCOSE, FASTING, PLASMA 203.
GLUCOSE, POST-PRANDIAL, PLASMA 221.
The sugar medicine needs to be stepped up. Good control is needed as you are now developing complications of diabetes. XXXXXXX XXXXXXX XXXXXXX nashini vati is probably not enough for your levels of sugars.
I would suggest changing to metformin 500mg once daily. Dose of the medicine will need to be increased slowly.
Repeat HbA1C at the end of 3 months of starting metformin.
Check sugars regularly at home with a glucometer and maintain a diary of these sugars.

3. CHOLESTEROL 181
TRIGLYCERIDES 256
HDL CHOLESTEROL 30
DIRECT LDL CHOLESTEROL 105
The lipid profile is abnormal. This, along with abnormal sugars can possibly spell a high cardiac risk. I would suggest statins like atorvastatin 20 mg at bedtime with good control of diet and moderate physical exercises. Repet test in 3 months of starting the medicine. Dose can be adjusted thereafter.

4. URINALYSIS WBC 5-7 , BACTERIA DETECTED (FEW).
This spells urine infection. Test a urine culture. Meanwhile start taking higher volumes of water and fluids. Antibiotics may be needed.

5. SODIUM 135.8.
This is ok. You need not worry about it.

6. As for occasional rashes, it might be allergic. If you can upload a picture of rashes, a better suggestion can be given.

7. With regards loosestools, I need to know the following details-
Does it happen with abdominal pain/nausea/vomit?
Is it preceded by constipation & pain?
Do you have outside food?
Sometimes, loosestools can develop due to excess bacterial growth in intestines due to high sugars. So good control of sugars is needed.

I have tried to make it as simple as possible.

Hi Sir,

I have started 1 tablet daily of Glycomet (500mg) after meal in daytime.
My question is whether I should continue taking the medicine after meal/before meal/during meal.
Also , Should I continue/discontinue my earlier XXXXXXX Madhunashini Vati(2 every day).
My today's sugar readings are:
At 7:15 a.m - 208
At 3:00 p.m - 207
At 10:00 p.m - 224

Hello.
First- glycomet is to be taken with meal.
Second- I wont suggest mixing the 2 systems of medicine.
Third- Sugar chart should be half hour before meal or 2 hours after meal.
About the sugar levels- the medicine needs to be continued. As I said before, the dose will need to be adjusted to your needs. And this needs to be done gradually. At least 2 weeks sugars are to be monitored before I suggest changing the dose of the medicine.
I have tried to make it as simple as possible.
