What Do My Lab Test Results Indicate?

Question: Doctor I am 22 years Female patient and my medical report shows 'impaired cortico medullary ecodifferention' with report impression of 'bilateral renal parenchymal disease'.
My creatnine rating is 5 which is outside of normal.

I have no high BP/diabetes etc.

Pls suggest is this is curable and different ways of treatment.

I have seen the reports attached by you. It looks like there is loss of corticomeduallary differentiation. However this is highly unikely in a 22 year old especially if there is no prior history of diabetes or hypertension. I would like you to get back to me with the following details

1. Could you please furnish the blood tests as well? Was urine routine done?
2. What were your symptoms? Why was the ultrasound done?
3. Do you have any swelling of feet or loss of appetitie ?
4. Have you bee taking any alternative forms of medicine, pain killers, etc for a long duration?

Please get back to me, I am awaiting your reply

Dear Dr Madhyastha, Thanks for the reply.

1) I am attaching all blood and urine tests reports done last week.
2) Patient about two weeks ago complained of lower back pain and hence some tests were done which showed Creatinine rating of 4.9mg/100ml. This was tested twice to confirm as the results were surprising. Once confirmed of Creatinine, that lead to more tests and reports are attached.
3) there is no swelling on feet and appetite is normal.
4) no medicines are administered as of now.

I wanted to know if this is fatal/fully curable and what kind of medical intervention is possible.

Hello

Thanks for attaching the reports. It looks like you have renal impairement with no obvious cause. Here is what you should do

1. Low hemoglobin is because of iron deficiency. It should be treated with oral iron therapy like Tab Orofer xt twice a day
2. High phosphorus has to be tackled as well. Since your calcium is low normal I recommend Tab Calcium acetate 667 twice a day along with food. This will increase the calcium as well as reduce the phosphorus
3. Your kidney size is normal and so is the urine routine apart from mild protein loss. I strongly suggest you to get a renal biopsy done as soon as possible to look for any reversible causes for kidney injury. I also recommend you to get ANA IF and dsDNA levels done to rule out Systemic lupus. One can know if it is curable only if biopsy is done so please appraoch a nephrologist as soon as possible
4. This is not a fatal illness. Most often it can be controlled with medications and if the renal failure progresses then it may require renal replacement therapy however that is only at a later stage

I did consult a nephrologist. As per him both kidneys have damaged >90% and the only option available is kidney transplant.

Pls suggest if you see the same. And which place you think is best for transplant.

Thanks for getting back. It might be true that 90% of the renal cortex is damaged however since the history is short and the kidneys are still normal in size one can attempt a biopsy to confirm it.
Please mention the treatment she is on so that I can give a second opinion on that
